pharmachologic effect

Erius is an antihistamine - a drug that blocks histamine receptors in the body, which leads to inhibition of allergic reactions. The active substance of Erius selectively acts on neurotransmitters that influence the blistering-hyperemic reaction of the skin, itching, etc., without penetrating the blood-brain barrier. Research has proven that the use of Erius can suppress the following body reactions:

  • Production of pro-inflammatory chemokines and cytokines;
  • Eosinophilic granulocyte adhesion and chemotaxis;
  • IgE-dependent release of histamine, prostaglandin D2 and leukotriene C4;
  • Increased adhesiveness of P-selectin;
  • Development of acute bronchospasm-atopic nature (in experiments involving animals).

The drug has antipruritic, antiallergic and antiexudative effects, preventing the occurrence and easing the course of allergic reactions. Erius effectively relieves spasms of smooth muscles and prevents the appearance of tissue swelling.

Reviews of Erius do not note the appearance of a pronounced sedative (calming) effect. In addition, clinical studies have shown that taking Erius at a dosage of 20 mg/day for 2 weeks does not have a significant effect on the functioning of the cardiovascular system. Unlike other antihistamines, the effectiveness of Erius is not affected by the consumption of fatty foods or grapefruit juice.

Indications for use of Erius

According to the instructions, Erius is effective against diseases such as:

  • Allergic rhinitis (including intermittent and persistent);
  • Hay fever, intended for quick relief (elimination of painful phenomena) itching in the eyes, sneezing, lacrimation, coughing, swelling of the nasal mucosa, conjunctival hyperemia;
  • Chronic idiopathic urticaria.

Contraindications

The use of Erius is not recommended for persons who are hypersensitive to desloratadine and other components included in the drug. In addition, it is not recommended to take Erius if you have congenital fructose intolerance or the presence of diseases associated with impaired absorption of galactose or glucose - due to the presence of these substances in Erius syrup.

At the moment, the safety profile of Erius for pregnant women has not been established, so taking the drug by women expecting a child is not recommended. It has been clinically discovered that the active substance of the drug passes into breast milk, so breastfeeding should be stopped while taking Erius.

Erius should be used with caution in patients suffering from severe renal failure.

Side effects of Erius

Side effects of Erius and analogues of the drug include increased fatigue, dry mouth, headache, tachycardia, allergic reactions (anaphylactic shock, rash), hepatitis, increased activity of liver enzymes, increased bilirubin in the blood. In reviews of Erius, cases of insomnia, diarrhea and chills were noted when taking the drug by children under 2 years of age.

How to use Erius

The optimal dosage of Erius according to the instructions for adults and adolescents from 12 years of age is 5 mg per day. It is not recommended to break the tablet; the medicine is taken whole with a small amount of liquid. Dosage for children from 2-5 years: 2.5 ml of syrup, for children from 6-11 years: 5 ml of syrup.

Overdose

If the dosage of the drug is exceeded, it is recommended to carry out standard measures to remove desloratadine from the gastrointestinal tract (taking activated charcoal, gastric lavage) and carry out symptomatic therapy. The drug is not eliminated by hemodialysis. According to specialist research, an overdose of the drug by 9 times does not have a serious negative effect on the body.

Analogues of the drug

Analogues of Erius (substitute drugs) are such drugs as Desloratadine, Desloratadinagemisulfate, Lordestin.

Other information

Eating at the same time as taking Erius does not have a significant effect on the effectiveness of the drug.

Pharmacotherapeutic group

Pharmacological properties

Long-acting non-sedating antihistamine. It is the primary active metabolite of loratadine. Selectively blocks the activity of peripheral H1 histamine receptors. Suppresses the release of histamine from mast cells. Inhibits the cascade of allergic inflammation reactions, including the release of pro-inflammatory cytokines, including interleukins IL-4, IL-6, IL-8, IL-13, and the release of adhesion molecules such as P-selectin. Thus, it prevents the development and facilitates the course of allergic reactions, has antipruritic and antiexudative effects, reduces capillary permeability, and prevents the development of tissue edema and smooth muscle spasm.

The drug has no effect on the central nervous system, does not cause drowsiness (the use of desloratadine at the recommended dose of 5 mg per day is not accompanied by an increase in the incidence of drowsiness compared to the placebo group) and does not affect the speed of psychomotor reactions. Does not cause prolongation of the QT interval on the ECG.

Desloratadine is well absorbed from the gastrointestinal tract. Determined in blood plasma 30 minutes after oral administration. The maximum concentration is reached on average 3 hours after administration. Does not penetrate the blood-brain barrier. Communication with plasma proteins is 83-87%. When used in adults and adolescents for 14 days at a dose of 5 mg to 20 mg once a day, no clinically significant accumulation of the drug was observed. Concomitant ingestion of food or grapefruit juice does not affect the distribution of desloratadine when used at a dose of 7.5 mg once daily. Desloratadine is not an inhibitor of CYP3A4 or CYP2D6 isoenzymes and is not a substrate or inhibitor of P-glycoprotein. Intensively metabolized in the liver by hydroxylation to form 3-OH-desloratadine combined with a glucuronide. Only a small part of the dose taken orally is excreted unchanged by the kidneys (<2%) and intestines (<7%). The half-life is an average of 27 hours. Based on single-dose, crossover studies, desloratadine 5 mg lozenge and conventional 5 mg tablet formulations are bioequivalent. The effectiveness of desloratadine 2.5 mg tablets has not been studied in pediatric patients. However, pharmacokinetic data obtained from a dose-finding study in pediatric patients demonstrate the feasibility of desloratadine 2.5 mg lozenges in children aged 6 to 11 years.

Water does not affect the bioavailability of the drug Erius ® lozenges.

Indications for use

Contraindications

In the presence of severe renal failure, Erius ® should be used with caution.

Each dose of the drug containing 5 mg of the active substance contains 2.9 mg of phenylalanine. Phenylalanine may cause unwanted effects in patients with phenylketonuria.

Adults and adolescents 12 years of age and older: One 5 mg lozenge once daily to reduce symptoms of allergic rhinitis (including seasonal and perennial allergic rhinitis) and urticaria.

Children ages 6 to 11 years: One 2.5 mg lozenge once daily to reduce symptoms of allergic rhinitis (including seasonal and perennial allergic rhinitis) and urticaria.

For seasonal (intermittent) allergic rhinitis (symptoms lasting less than 4 days a week or less than 4 weeks a year), the course of the disease must be assessed. If symptoms disappear, the drug should be stopped; if symptoms reappear, the drug should be resumed.

For year-round (persistent) allergic rhinitis (if symptoms last more than 4 days a week or more than 4 weeks a year), the drug should be taken throughout the entire period of allergen exposure.

Side effect

Very rarely (<1/10,000) the following undesirable effects were observed:

  • from the nervous system: hallucinations, dizziness, drowsiness, insomnia, psychomotor hyperactivity, convulsions;
  • from the liver and biliary tract: increased activity of liver enzymes, bilirubin concentration, hepatitis;
  • from the digestive system: abdominal pain, nausea, vomiting, dyspepsia, diarrhea;
  • from the cardiovascular system: tachycardia, palpitations;
  • from the musculoskeletal system: myalgia;
  • allergic reactions: anaphylaxis, angioedema, shortness of breath, itching, rash, including urticaria.

    Overdose

    In a clinical pharmacological study, when using desloratadine at a dose of up to 45 mg (9 times higher than the therapeutic dose), no clinically significant adverse effects were observed.

    Desloratadine is not removed by hemodialysis. There are no data on its elimination during peritoneal dialysis.

    Information regarding acute overdose is limited to post-marketing reports of adverse events and data from pre-marketing clinical studies. In a dose-finding study, drowsiness was reported with desloratadine 10 mg and 20 mg daily. Interaction with other drugs Interactions with other drugs were not detected in studies with azithromycin, ketoconazole, erythromycin, fluoxetine and cimetidine. Eating does not affect the effectiveness of the drug. Erius ® does not enhance the effect of alcohol on the central nervous system.
