Overdose

Symptoms of overdose are caused mainly by paracetamol and appear after taking more than 10–15 g.

In acute overdose, paracetamol has a hepatotoxic effect, including causing liver necrosis.

Overdose may cause nephropathy with irreversible liver failure. The severity of an overdose depends on the dose, so patients should be warned not to take drugs containing paracetamol at the same time. The risk of poisoning is pronounced especially in elderly patients, in children, in patients with liver disease, in cases of chronic alcoholism, in patients with chronic malnutrition (calorie deficiency) and in patients taking inducers of microsomal oxidation in the liver. Overdose of paracetamol can lead to impaired glucose metabolism, hypoglycemia, bleeding, pancreatitis, liver failure, encephalopathy, coma and death.

Symptoms of paracetamol overdose in the first 24 hours: pale skin, nausea, vomiting, anorexia, convulsions. Abdominal pain may be the first sign of liver damage and usually does not appear for 24 to 48 hours and may sometimes appear later, after 4 to 6 days, on average 72 to 96 hours after taking the drug. Treatment includes surgical gastric lavage, symptomatic and supportive therapy. The hypertensive effect can be reversed by intravenous administration of an alpha-receptor blocker. In case of seizures, diazepam may be used.

Symptoms of chlorphenamine maleate overdose include drowsiness, respiratory arrest, seizures, anticholinergic effects, dystonic reactions, and cardiovascular collapse including arrhythmia.

In children, symptoms of overdose may include incoordination, agitation, tremors, behavioral changes, hallucinations, seizures, and anticholinergic effects.

Treatment includes gastric lavage in case of massive overdose, or induction of vomiting. After this, it is possible to prescribe activated charcoal and a laxative to slow down absorption. In case of seizures, sedation should be performed with intravenous diazepam or phenytoin. In severe cases, hemoperfusion may be performed.

Interaction with other drugs

Enhances the effects of MAO inhibitors, sedatives, ethanol. The risk of hepatotoxic action of paracetamol increases with simultaneous use of barbiturates, phenytoin, phenobarbital, carbamazepine, rifampicin, isoniazid, zidovudine and other inducers of microsomal liver enzymes. The anticoagulant properties of warfarin and other coumarins may be enhanced by long-term regular use of paracetamol, increasing the risk of bleeding. A single dose of paracetamol does not have this effect. Metoclopramide increases the rate of absorption of paracetamol and increases the level of paracetamol concentration in the blood plasma to the maximum. Likewise, domperidone may increase the rate of absorption of paracetamol.

When chloramphenicol and paracetamol are used together, the half-life of chloramphenicol may increase.

Paracetamol may reduce the bioavailability of lamotrigine, with a possible decrease in its effect due to induction of its hepatic metabolism. The absorption of paracetamol may be reduced when taken concomitantly with cholestyramine, but this can be avoided if cholestyramine is taken one hour after paracetamol. Regular use of paracetamol concomitantly with zidovudine may cause neutropenia and increase the risk of liver damage. Probenecid affects the metabolism of paracetamol. In patients taking probenecid concomitantly, the dose of paracetamol should be reduced. The hepatotoxicity of paracetamol may be increased by chronic or excessive alcohol consumption. Paracetamol may interfere with the results of the uric acid test using the phosphotungstate precipitating reagent.

Contraindicated in patients who are taking or have taken MAOIs within the past two weeks. Phenylephrine can enhance the effect of MAO inhibitors and cause a hypertensive crisis.

Concomitant use of phenylephrine with other sympathomimetic drugs or tricyclic antidepressants (eg, amitriptyline) may increase the risk of cardiovascular side effects. Phenylephrine may reduce the effectiveness of beta blockers and other antihypertensive drugs (eg, debrisoquine, guanethidine, reserpine, methyldopa); The risk of increased blood pressure and other cardiovascular side effects may be increased. Concomitant use of phenylephrine with digoxin and other cardiac glycosides may increase the risk of arrhythmia or myocardial infarction. Antihistamines, such as chlorphenamine, may enhance the effects of opioid analgesics, anticonvulsants, antidepressants (tricyclics and monoamine oxidase inhibitors), other antihistamines, antiemetics and antipsychotics, anxiolytics, hypnotics, ethanol (alcohol) and other central nervous system depressants.

Because chlorphenamine has some anticholinergic activity, the effects of anticholinergic drugs (eg, some psychotropic drugs, atropine, and urinary incontinence drugs) may be increased by this drug. Coldact dosage

The medicine is intended for oral administration. Adults are prescribed to take 1 capsule about two times a day. Thanks to paracetamol, which has analgesic and antipyretic properties, it reduces pain symptoms that appear during colds, such as sore throat, headache, muscle and joint pain, and lowers temperature. Unlike paracetamol, phenylephrine has vasoconstrictor properties, which reduce swelling and hyperemia of the mucous membranes of the respiratory tract and in the paranasal sinuses.

Coldact Flu Plus is used in the symptomatic treatment of colds, in the treatment of influenza, ARVI.

The drug is produced in the form of syrup, which is a combination product. Thanks to its constituent Guaifenesin and Ambroxol, which are mucolytic substances, it can reduce the viscosity of bronchial secretions and facilitate expectoration of sputum. In addition, chlorphenamine, which is part of it, has an antihistamine effect and is able to eliminate itching of the mucous membranes of the nose and eyes, eliminate bleeding, and phenylephrine, an alpha1-adrenergic stimulant, has vasoconstrictor properties and is able to reduce swelling and hyperemia of the mucous membranes of the respiratory tract and paranasal sinuses.

The medicine is used for the symptomatic treatment of colds, influenza, ARVI, which are accompanied by a severe cough and difficult-to-discharge sputum of a viscous consistency.

For the treatment of adults and children over the age of 12 years, 20 ml of syrup is prescribed about three times a day. Children aged 6-12 years are prescribed 10 ml of syrup about three times a day. The duration of treatment is usually 7 days.

The drug is an antiseptic and is available in the form of tablets with different flavors. This medicine has anti-inflammatory, analgesic and local anesthetic effects. Koldakt is a powerful combination drug for colds and allergies. It is intended to relieve or reduce the severity of clinical symptoms, and does not in any way affect the cause of the pathology. The components of this medicine have vasoconstrictor (vasoconstrictor), anticongestive (decongestant) and antihistamine (antiallergic) properties.

Active ingredients and dosage forms

Another active substance, phenylpropanolamine, is an indirect-acting sympathomimetic. Its action is similar to that of ephedrine, but it affects the central nervous system to a lesser extent. The compound helps reduce congestion in the mucous membranes of the upper respiratory system.

Coldact stimulates α-adrenergic receptors and constricts blood vessels, thereby reducing hyperemia and swelling of the mucous membranes of the respiratory tract and nasal sinuses.

Paracetamol inhibits the enzyme cyclooxygenase and affects the hypothalamic centers of thermoregulation and pain. This compound helps reduce high fever and relieve headaches and muscle pain during colds and acute respiratory viral infections (including influenza).

After oral administration of Coldact capsules, the active components are absorbed gradually in the digestive tract. The maximum serum concentration of chlorpheniramine is recorded after 2-6 hours. Its biotransformation occurs in the liver, and the unchanged substance and its metabolites are excreted mainly by the kidneys.
