PHARMACOTHERAPEUTIC GROUP: antibiotic for topical use

Mechanism of action. Mupirocin is a local antibiotic. Mupirocin inhibits isoleucyl transfer RNA synthetase, disrupting protein synthesis in the bacterial cell.

Due to its specific mechanism of action and unique chemical structure, mupirocin does not show cross-resistance with other antibiotics.

When mupirocin is used correctly, the risk of the emergence of resistant strains of microorganisms is small. When used in minimal inhibitory concentrations, mupirocin has bacteriostatic properties, and in higher concentrations, ¾ bactericidal properties.

Antimicrobial activity: in vitro effective against Staphylococcus aureus (including methicillin-resistant strains), Staphylococcus epidermidis, Streptococcus species, and also against gram-negative microorganisms: Haemophilis influenzae and Escherichia coli. Other gram-negative microorganisms are less sensitive, and strains of Pseudomonas aeruginosa are resistant to mupirocin.

Efficacy against the listed microorganisms has been clinically confirmed.

Pharmacokinetics. Mupirocin, after intranasal administration, enters the systemic circulation when used in newborns and premature infants, without causing any undesirable effects indicating intolerance to the drug. Absorption of mupirocin through the mucosa (less than 1% of the applied dose is excreted by the kidneys in the form of miniic acid).

Local infections of the nasal cavity caused by microorganisms sensitive to mupirocin, including carriage of Staphylococcus aureus (including methicillin-resistant strains).

Known hypersensitivity to any of the components of the drug.

USE IN PREGNANCY AND LACTATION

There are no data on the use of the drug during pregnancy and breastfeeding. However, preclinical studies of the drug did not reveal any negative effects on pregnancy or embryonic development.

The ointment is applied using an applicator (reusable tube). After treating the surface of the mucous membrane, the nasal passages should be tightly compressed, which ensures uniform distribution of the ointment throughout the nasal mucosa.

The course of treatment is 3 – 5 days. The drug should not be used for more than 10 days without reassessing the condition. It should not be mixed with other medicinal products for intranasal use to avoid dilution of the ointment and, consequently, a decrease in antibacterial activity or a possible change in the stability of the intranasal ointment.

Dosage regimen. Apply a small amount of ointment, approximately the size of a match head (approximately 30 mg ointment), into each nasal passage twice a day for five days.

In 12 clinical studies, the incidence of adverse reactions was less than 0.1%.
