A double-blind, placebo-controlled, randomized clinical trial was conducted on the effectiveness and safety of Anaferon for children in the treatment of chickenpox in children. 136 children aged 1 to 18 years with characteristic clinical manifestations of chickenpox from 1-2 days of illness (from the appearance of the rash) until recovery (up to 10 days) received Anaferon for children; the placebo group consisted of 100 children. Clinical effectiveness of using infantine Anaferon in treating children/infants suffering from Chickenpox

The investigators have made a dual placebo-controlled randomized clinical study on the effectiveness and safety of application of infantine Anaferon in treating children/infants suffering from chickenpox. One hundred and thirty six infants, children and youngsters aged one to eighteen, who exhibited characteristic manifestations of chickenpox, were administered infantine Anaferon from the 1st or 2nd day they were taken ill (appearance of rash) till recovery (up to ten days). One hundred patients made up the placebo group. Administered infantine Anaferon reduced the ailment duration on the average by 2.7 days, the term of appearance of new eruptions by 3.3 days and itch by 4.2 days. Administration of infantine Anaferon has revealed a greater number of instances of abortive (atypical) progress of chickenpox. The administered preparation diminished the risk of development of pustules 6.5 times and demand for additional antibacterial treatment 9.1 times. Infantine Anaferon was adequately tolerated by the patients and went well together with symptomatic therapy. Key words: chickenpox, children/infants, viral infections, infantine Anaferon

According to the Federal Center for State Sanitary and Epidemiological Surveillance, the incidence of chickenpox in children in 2004 was 459 per 100 thousand, and the mortality rate from chickenpox was 100 thousand cases [1]. The issue of effective and safe specific etiotropic therapy for chickenpox in children still remains unresolved. There are a number of chemotherapy drugs for the treatment of viral diseases that can have a direct antiviral effect (rimantadine, acyclovir, Zovirax, arbidol, etc.). However, these drugs are not sufficiently effective in the treatment of chickenpox, cause a variety of side effects (gastrointestinal, pulmonary, hematological, etc.), and do not reduce the risk of complications [2].

It is well known that the leading role in the body’s antiviral defense against infections belongs to the interferon (IFN) system—cytokines that have universal antiviral properties [3].

In recent years, there has been an active search for new drugs that act on the interferon system, which, along with an antiviral effect, have immunomodulatory activity.

The spectrum of antiviral activity of interferon drugs and interferon inducers is wide: influenza and other acute respiratory viral infections, herpetic infections, AIDS, mumps, cytomegalovirus infection, hepatitis, hemorrhagic fever with renal syndrome, infectious mononucleosis, etc. [4]

A representative of this group of drugs is Anaferon for children, containing ultra-low doses of antibodies to γ-interferon - a mixture of homeopathic dilutions C12, C30 and C50. According to previously obtained results [5], ultra-low doses of antibodies have a modifying effect in relation to the antigen (in this case, interferon γ), they regulate and enhance its activity.

In an experimental study of the antiviral activity of Anaferon for children, it was shown that Anaferon for children has an antiviral effect when administered prophylactically and therapeutically in conditions of experimental infections - influenza and herpes, stimulates the humoral and cellular immune response, increases the phagocytic activity of neutrophils, macrophages and the spontaneous production of endogenous interferon gamma by lymphocytes , increases the functional activity of T-helpers. In vitro, Anaferon for children increases the functional activity of T-, B-lymphocytes and NK-cells [6-8].

Preclinical toxicological studies of Anaferon for children, including the study of acute and chronic toxicity, allergenic properties, cytogenetic effects, reproductive toxicity (including embryotoxic and teratogenic effects), did not reveal the presence of any of the above types of toxicity in the drug [9]. Moreover, Anaferon for children had a moderate antimutagenic effect.

Clinical studies of the effectiveness and safety of Anaferon for children against influenza and other acute respiratory viral infections in children were conducted at the Influenza Research Institute of the Russian Academy of Medical Sciences (St. Petersburg), the Russian State Medical University (Moscow), and the Volgograd State Medical University [10-15]. In the group of children receiving Anaferon for children, a significant reduction in the severity and duration of the main clinical manifestations of viral infections, as well as complications, a reduction in the duration of intoxication, fever and rhinitis and the length of stay of children in the hospital was registered. A positive effect of Anaferon for children on interferon status was revealed: the use of the drug contributed to a pronounced induction of endogenous interferons on days 2-3 of treatment and prevented a decrease in the production of interferon by lymphocytes at the stage of convalescence. There was a restoration of normal levels of lymphocytes of various subpopulations in the peripheral blood (CD3+, CD4+, CD8+, CD20+, CD16+), a decrease in the level of IgE and an increase in IgA.

Pilot studies of the effectiveness of Anaferon for children in the treatment of infectious mononucleosis [16] (IM) were conducted at the Siberian State Medical University (Tomsk) and the Ural State Medical Academy (Ekaterinburg). The average duration of fever and sore throat (the most typical complication) was 2.4 and 4.3 days in the Anaferon for children group, and 2.2 and 2.8 days in the group receiving combination therapy (Anaferon for children + acyclovir) and was significantly shortest .

A pilot open comparative study of the effectiveness of Anaferon in hemorrhagic fever with renal syndrome (HFRS) in children was conducted at the Bashkir State Medical University (Ufa) [17]. The use of Anaferon for children reduced the duration of the febrile period by an average of 3.3 days; reduce the incidence of hemorrhagic syndrome (nosebleeds) by 3 times compared to the placebo group, and the incidence of renal syndrome by 5 times. Complete relief of clinical and laboratory manifestations of HFRS in the AF group occurred, on average, in 11.5 days, which was 3.7 days earlier than the comparison group.

Taking into account the convincing data obtained on the effectiveness of the use of Anaferon for children as a treatment for viral infections in children, including herpesvirus infections, it seemed promising to conduct a clinical study of the effectiveness and safety of Anaferon for children in the treatment of chickenpox.

Materials and research methods

A double-blind, placebo-controlled, randomized study was conducted with a treatment duration of up to 10 days.

Using a random sampling method, 2 groups of patients were formed: the first group (136 people) took Anaferon for children (RU No. 000372/dated 04/05/2001, LLC NPF Materia Medica Holding), the second (100 people) took a placebo drug.

Treatment regimen: in the first 2 hours - every 30 minutes; then during the first day 3 more times at equal intervals; from day 2 onwards - 3 times a day until recovery. The drug was prescribed in combination with local symptomatic therapy (shading of the rash elements with a 1% alcohol solution of brilliant green or an aqueous solution of potassium permanganate; rinsing the mouth with an aqueous solution of furacillin). The use of immunomodulators and antiviral agents was not allowed; antipyretics were prescribed according to indications and, if indicated, antibacterial therapy was prescribed.

The timing of normalization of body temperature was assessed daily; maximum number of rashes; the date of appearance of the last new rashes; term for cessation of itching, need for antibacterial therapy. A summary assessment of the severity of the disease was also carried out, and the frequency and severity of complications were calculated. The severity of the condition was assessed according to the classification proposed by V.F. Uchaikin [20].

Safety was assessed by the timing and nature of adverse events, association with the study drug, and the need for additional treatment to relieve adverse events.

Mean values ​​and error of the mean were calculated. The average duration of symptoms of the disease was compared with the placebo group (Student's t test for independent samples). Proportions of patients were compared using the Chi-square test.

Results and its discussion

At inclusion in the study, both groups were homogeneous in terms of demographics and severity of the condition.

The Anaferon children's group included 136 children, including 71 (52.2%) boys, 65 (47.8%) girls. At the first visit, in 62 (87.3%) boys the disease was assessed as a mild form, and in 9 (12.7%) - moderate severity. In 49 (75.4%) girls, the disease was mild at the beginning, in 14 (21.5%) it was moderate, and in 2 (3.1%) it was severe (Table 1).

The placebo group included 61 (61%) boys and 39 (39%) girls. The form of severity on days 1-2 of the disease was mild in 48 (78.6%) boys, and moderate in 13 (21.3%) boys. The initial severity level was mild in 26 (66.7%) girls, moderate in 11 (28.2%) and severe in 2 (5.1%) (Table 2).

Tables 3 and 4 provide summary data on the assessment of the severity and severity of fever in children of the study groups at the onset of the disease.

Thus, the groups of children participating in the study were homogeneous in gender, age and severity.

Dynamics of body temperature and intoxication syndrome. In both compared groups, the majority of patients had a febrile condition: 95.6% in the Anaferon children’s group and 84% in the placebo group (Table 4); the time frame for normalization of body temperature differed significantly between them. Thus, in the main group, in 133 (97.8%) patients, normalization of body temperature was noted during the first three days, while in the control group, in 80 (80%) patients, normalization of body temperature was noted on days 4-6. Only 4 (4%) patients in the placebo group had body temperature normalized by day 3 of observation (Table 5). It should be emphasized that Anaferon for children was effective regardless of the initial severity of the condition. Thus, out of 111 patients in the Anaferon group for children with an initially mild form of the disease, in 105 (94.5%) patients the body temperature returned to normal on days 2–3 of the disease. In the placebo group of 74 patients with an initially mild form of the disease, body temperature returned to normal on days 4–6 in 67 (90.5%) people, and only in 4 (5.4%) on days 3.

In all 23 patients of the Anaferon children's group with an initial moderate form of chickenpox, the body temperature returned to normal on days 2-3 (100%), while in 24 patients with a moderate form of the placebo group, the temperature returned to normal no earlier than 5 days (12.5% ​​- 3 person), and in the remaining majority of patients with a moderate form from the placebo group (75% -18 people), the temperature returned to normal on days 6-7 of the disease.

Maximum number of rashes. In both groups, the number of rashes in the majority of patients was: 81 patients (59.6%) in the main group, 73 (73%) in the placebo group. It is noteworthy that during the entire observation period in the Anaferon children's group, the proportion of patients with the number of rashes less than 50 was significantly higher - in 25 (18.4%) versus 1 (1%), and with the number of rashes more than 200 - significantly lower 30 (22.8%) versus 33 (39%) in the placebo group (Table 7).

The influence of therapy on the dynamics and nature of rashes. In Fig. Figure 2 shows the dynamics of skin manifestations in the Anaferon for children and placebo groups, corresponding to the dynamics of fever.

The effect of therapy with Anaferon for children on the timing of extinction and the characteristics of the course of skin rashes is illustrated in Tables 8 and 9. It is noteworthy that in 80 (58.8%) patients who received Anaferon, the maximum “addition” of new elements occurred on the 2nd day, and in the control group group in 68 (68%) patients, the same symptom persists on day 6. The typical form of chickenpox is characterized by vesicular-type rashes on the skin and mucous membranes with or without an umbilical depression in the center, but with the obligatory presence of subsequent stages: drying out, crusting, falling off of the crust. Atypical chickenpox is characterized by skin rashes in the form of "resi", spots, papules and papules without the formation of a crust and its falling off. During treatment with Anaferon for children, a decrease in the number of cases of typical chickenpox was recorded to 77.2% compared to the control group (98%). Accordingly, during treatment with Anaferon, the number of atypical cases was registered in 22.8% of children, in the control group - in 2%. The differences in the dynamics of atypical elements of chickenpox rash are illustrated in Fig. 4. It can be seen that the dynamics of atypical elements was slowed down in the placebo group compared to the main one.

Skin itching. The dynamics of skin itching are presented in Table. 10. It can be seen that in the main group the cessation of itching was observed in 135 (99.3%) patients during the first 3 days, while in 36 (26.5%) patients itching did not develop at all. At the same time, in the control group, cessation of itching was noted in 89 (89%) patients on days 5–7, and it was absent in only 1 (1%) patient. Formation of pustules and the need for antibacterial therapy. It is interesting that in the group of patients who received Anaferon for children, there was a significantly less pronounced tendency to form pustules. The latter were observed in 19 (14%) patients of the main group, while in the control group the formation of pustules was registered in 96 (96%) patients (p The above is confirmed by a decrease in the need for antibacterial therapy in the Anaferon children's group. The inclusion of Anaferon in the complex treatment of chickenpox on days 1-2 of the disease significantly reduces the duration and severity of the disease, promotes more rapid elimination of nonspecific manifestations of the infectious process (fever and intoxication), as well as a more rapid (in some cases abortive) course of skin manifestations and risk reduction bacterial complications.

The use of Anaferon for children according to the treatment regimen significantly reduced the duration of the main clinical manifestations of chickenpox in children aged 1 to 17 years compared to the placebo group. Taking the drug reduced the duration of fever by 2.7 days, shortened the time of appearance of new rashes by 3.3 days, and skin itching by 4.2 days. During therapy with Anaferon for children, a greater number of cases of abortive (atypical) course of chickenpox were noted. The use of the drug also reduced the risk of developing pustules by 6.5 times and the need for additional antibacterial therapy by 9.1 times. Anaferon for children was well tolerated by patients and was combined with symptomatic therapy. There were no adverse reactions or subjective complaints associated with taking the drug, and no allergic reactions were registered.

The data obtained allow us to consider Anaferon for children as an effective and safe means of specific treatment of chickenpox in children from 1 to 17 years of age.

  • Ergoferon - despite the fact that it has an antiviral and antihistamine effect, it is prescribed to children very rarely. It is usually used in cases where various allergic reactions are involved.
  • Anaferon (Anaferon) is a homeopathic medicine. Such tablets are prescribed for chickenpox in children as an adjuvant against the background of general therapy for the disease. Reduces the risk of complications and speeds up the recovery process. Anaferon for chickenpox in children is an effective pill that speeds up the healing process.
  • Acyclovir is an antiviral drug based on interferon. It contains proteins similar to substances secreted by the body to eliminate the virus. Children under 3 years of age should not take the drug.

Miramistin is also available in the form of an ointment. This dosage form can be used only three times a day. The composition is used as an additional drug.

Calamine

Can be purchased as a cream or lotion. This product is based on natural ingredients. For external use only. The drug is aimed at relieving itching, swelling, eliminating redness and inflammatory foci. It disinfects and protects watery formations from mechanical stress that can be caused by clothing. Accelerates the healing of pockmarks. The composition is not capable of causing allergies. There are no age restrictions, can be used even at the age of 1 year.
