Erespal syrup side effects

Erespal instructions

Erespal is a bronchodilator and anti-inflammatory drug intended for the treatment of inflammatory diseases of the upper and lower respiratory tract, accompanied by severe inflammation, edema and bronchospasm.

Table of contents:

It contains an active substance - fenspiride hydrochloride, which belongs to the pharmacological group - H1-antihistamines and is therefore an anti-inflammatory agent, with an antihistamine, myotropic antispasmodic and antiexudative mechanism of action and has a bronchodilator effect. Therefore, this drug is one of the drugs included in regimens for the complex treatment of bronchial asthma.

Phakmacodynamics of Erespal

The mechanism of action of erespal is its pronounced anti-inflammatory, anti-edematous and antispasmodic effect on the mucous membrane of the upper and lower respiratory tract, as well as its anticonstrictor effect on the bronchial tree. Its action is based on active blockade of H1-histamine receptors, inhibition of arachidonic acid metabolism and blockade of alpha-adrenergic receptors. The effect of fenspiride on arachidonic acid metabolism occurs indirectly through calcium metabolism, which is a regulator of the activity of phospholipase A, which promotes the formation of arachidonic acid. Thus, it antagonizes the mediators of allergy and inflammation - histamine, serotonin and bradykinin, reduces the production of leukotrienes and prostaglandins, which are also stimulated by histamine, and reduces the secretion of bronchial glands and has a chemotactic effect. When used in high doses, fenspiride significantly inhibits the production of inflammatory agents in the mucous membranes: free radicals, cytokines, tumor necrosis factor alpha and arachidonic acid metabolites. Some of these inflammatory factors have a bronchoconstrictor effect, so a decrease in their production causes an antiasthmatic and bronchodilator effect. In this regard, the activity and release of a number of factors that contribute to the development of inflammation, hypersecretion of inflammatory agents and bronchial obstruction decreases. It also has a paperrine-like myotropic antispasmodic effect.

Fenspiride in its mechanism of action differs from all anti-inflammatory drugs (non-steroidal and steroidal anti-inflammatory drugs), since it has an original effect on the inflammatory process and affects all inflammatory mediators.

Pharmacokinetics of Erespal

Erespal has good absorption from the gastrointestinal tract. Its maximum concentration in the blood plasma after taking the syrup is achieved after 2.3 hours, and when taken in tablet form - after 6 hours, on average from 0.5 to 8 hours and depends on the absorption capacity of the intestine. Erespal is excreted from the body 90% by the kidneys, and 10% through the intestines. The half-life of the drug from the body is 12 hours.

Instructions for use of Erespal

Due to its high efficiency, excellent anti-inflammatory and bronchodilator effects, this drug is used for most diseases of the upper and lower respiratory tract as an anti-inflammatory, bronchodilator, anti-edematous, antispasmodic agent.

Instructions for using erespal

This drug has two forms of release: in the form of syrup and tablets and is produced by Servier Laboratories in France.

The syrup is available in 150 ml dark plastic bottles containing 2 mg of the active substance fenspiride per 1 ml of syrup.

The tablet form of erespal is tablets of 15 pieces in a blister, film-coated and containing 80 mg of fenspiride.

The syrup is an orange transparent liquid, taken orally before meals, it is advisable to shake it before use.

The dose for adults and adolescents is: three to six tablespoons per day, which is ml. One tablespoon of syrup contains 30 mg of fenspiride.

For adults, Erespal tablets are prescribed 1 tablet (80 mg) two to three times a day.

For children under fourteen years of age, erespal is prescribed only in the form of syrup.

The recommended dosage is 4 mg/kg per day, which is:

For body weights up to ten kilograms - ml per day, from two to four teaspoons daily, the drug can be added to a bottle of baby food;

from two to sixteen years, with a weight of more than ten kilograms, the recommended dose is 2-4 tablespoons of syrup, which corresponds to 1 ml daily.

Contraindications

Contraindications for use are individual hypersensitivity to the active substance of the drug - fenspiride hydrochloride or another component included in the drug (most often to Sunset Yellow CFC dye or propylarbenam).

Taking the tablet form of the drug is also contraindicated for children under 14 years of age.

Special instructions and interactions with other drugs

This drug should be taken with caution in patients with fructose intolerance, with glucose-galactose maladsorption, in patients with diabetes mellitus and with isomaltose deficiency (due to the presence of sucrose in the drug).

It is not recommended to take the drug together with acetylsalicylic acid and other non-steroidal anti-inflammatory drugs due to the possibility of developing allergic reactions, including bronchospasm.

It is also not recommended to prescribe erespal together with a sedative group of drugs, since their interaction can lead to potentiation of the sedative effect and cause severe weakness and drowsiness (histamine H1 receptor blockers have their own sedative effect).

For the same reason, it is not advisable to combine the use of erespal and alcohol.

For inflammatory diseases of infectious origin, complicated by bacterial infection, erespal is prescribed in combination with antibacterial drugs.

Side effects of Erespal

Side effects of erespal include changes in the central nervous system in the form of drowsiness and dizziness;

from the cardiovascular system - palpitations and moderate tachycardia, which decreases with decreasing dose of the drug;

from the digestive system - gastrointestinal disorders, nausea, epigastric pain, sometimes vomiting and diarrhea;

Allergic reactions may also occur in case of individual sensitivity to the drug in the form of erythema, urticaria, angioedema, as well as asthenia, fatigue, and fixed erythema pigmentosa.

If any of these symptoms occur, you should immediately stop using Erespal and consult your doctor.

Overdose

An overdose of a drug occurs when the drug is taken in dosages exceeding the average daily allowable dose, when the drug accumulates in the body, which occurs when it is taken for a long time or when excretion is impaired.

Symptoms of an overdose of erespal are agitation or drowsiness, nausea, vomiting, and sinus tachycardia.

Sometimes situations arise when a small child drinks a large amount of the drug. In the case of erespal, this can be extremely dangerous due to its adrenergic blocking and histamine blocking effects, which can provoke an attack of sinus tachycardia, accompanied by severe weakness, drowsiness or overexcitation.

All organs and systems of the child’s body are characterized by ongoing differentiation, and therefore insufficient stability and a paradoxical reaction to a large simultaneous administration of the drug, and the delicate gastric mucosa contributes to faster absorption of the drug into the bloodstream. Therefore, if a situation arises where a child, out of ignorance or out of curiosity, takes a large dose of erespal, it is necessary to immediately contact a medical institution for medical help for gastric lavage and, if necessary, symptomatic drug therapy, detoxification measures and ECG monitoring.

Also no less dangerous when taking a large amount of the drug, both in children and adults, especially in old age, is the occurrence of allergic reactions in the form of angioedema or Quincke's edema, which are immediate-type allergic reactions and are characterized by a rapid course. Therefore, if they occur, you must immediately contact a medical facility.

Instructions for using erespal during lactation and pregnancy

Taking this medicine during pregnancy is not recommended, as there is always a risk of adverse effects of the medicine on the embryo (if used in the early stages) or on the developing fetus (in later stages of pregnancy). Some sources contain data on the teratogenic effect of fenspiride hydrochloride with the development of an anomaly of the upper palate in the form of a “cleft palate”. But therapy with erespal is not a basis for termination of pregnancy, since in general there is no clinical data on its teratogenic effect, and in this case the protective properties of the fetoplacental barrier and the individual characteristics of the course of pregnancy and the activity of the mother’s excretory systems should not be excluded.

Also, when breastfeeding a child, taking Erespal is not recommended, since the active components of the drug that enter breast milk can enter the child’s body and adversely affect its organs and systems, in particular causing increased drowsiness or overexcitation, as well as sinus tachycardia. Taking this drug is possible only if you stop breastfeeding for a while, due to the fact that studies on the concentration of the active substance in breast milk have not been conducted and for a child, especially a newborn, they may exceed the maximum permissible dose of the drug for his weight.

Practice of using erespal and reviews

Erespal is an excellent development of the pharmacological company Servier Laboratories, introduced in Russia back in 1998 and today is a very effective and popular drug that is widely used in medical practice - therapy, otolaryngology and pediatrics, as an anti-inflammatory and bronchodilator drug. It is prescribed for inflammatory diseases of the upper and lower respiratory tract of infectious and allergic origin, which are accompanied by spasm and severe cough - acute and chronic laryngitis, pharyngitis, laryngotracheitis, acute, chronic and obstructive bronchitis. It is also effective as a symptomatic remedy used in complex therapy for severe cough and spasm of the larynx and bronchi with whooping cough and parawhooping cough, spasmodic cough with measles and parainfluenza infection. Erespal has a potentiating anti-inflammatory effect when used together with antibacterial drugs for infectious and colds of the upper and lower respiratory tract, complicated by bacterial infection.

This drug is very effective in complex and maintenance therapy regimens for bronchial asthma, which is due to its dual mechanism of action at all stages of the development of the pathological process in this disease - bronchodilation, inhibition of mucus production, anti-edematous and anti-inflammatory effect, as well as inhibition of the production of histamine and serotonin, which potentiates its antiallergic effect.

Erespal is used in otolaryngology for the treatment of acute and chronic inflammatory processes of the paranasal sinuses and nasal cavity - sinusitis and rhinitis. It is used in combination with other drugs: antibacterial, desensitizing and local anti-inflammatory drugs. In this case, erespal, which has an anti-inflammatory effect on the mucous membrane of the paranasal sinuses and a stimulating effect on mucociliary transport, has proven itself in the treatment of purulent and pollipous sinusitis and rhinosinusitis.

Its effective practical use in combination with desensitizing drugs allows us to recommend erespal for the treatment of patients with combined chronic inflammation in the upper and lower respiratory tract, including bronchial asthma, subacute laryngotracheitis and exacerbations of chronic laryngitis.

In clinical use, the drug has excellent tolerability, even with long-term use.

Erespal storage conditions

Like all medications, erespal must be used only before its expiration date. It can be stored for no more than three years in a sealed form.

- diseases of ENT organs: rhinitis, nasopharyngitis, otitis, sinusitis;

— reduction of respiratory syndromes during acute respiratory infections, whooping cough, measles;

Erespal - has an anti-inflammatory effect, inhibits exudation, prevents bronchoconstriction. The anti-inflammatory effect of fenspiride is due to inhibition of arachidonic acid metabolism, and the antibronchoconstrictor effect is achieved by reducing the production of biologically active substances: serotonin, histamine (at the level of blockade of H1 receptors), bradykinin. It also blocks α-adrenergic receptors, the stimulation of which causes an increase in the production of bronchial secretions.

Adults and children over 14 years old: 1 tablet (80 mg) morning and evening or 3-6 tablespoons (45-90 ml) of syrup. If necessary, the daily dose can be increased to 320 mg, dividing it into 2-3 doses.

Intolerance to any of the components of the product.

From the gastrointestinal tract: nausea, vomiting, discomfort in the epigastric region.

Allergic reactions: more often urticaria, less often Quincke's edema. Usually associated with taking syrup, due to the presence of para-hydroxybenzoate in it.

Other side effects: drowsiness, slight tachycardia. In cases of tachycardia, it is necessary to reduce the dose of the product.

During pregnancy, Erespal is not prescribed, since there is no convincing clinical evidence of the absence of terratogenic and fetotoxic effects. At the same time, taking fenspiride is not an indication for terminating a pregnancy that occurs while taking it.

During Erespal therapy, it is necessary to interrupt breastfeeding, due to the lack of data on the excretion of fenspiride in breast milk.

Symptoms of overdose: nausea, vomiting, agitation, tachycardia, drowsiness. There are no special antidotes; in cases of overdose, gastric lavage, symptomatic therapy, and ECG monitoring are recommended.

The interaction of Erespal with other medicinal products has not been described.

Film-coated tablets, white No. 30.

Syrup 150 ml, 200 mg/100 ml, No. 1.

The drug should be stored at room temperature. The shelf life of Erespal in syrup form is 3 years, in tablet form - 2 years.

Each tablet contains 80 mg of fenspiride hydrochloride and excipients: calcium hydrogen phosphate, hypromellose, povidone, silicon dioxide, magnesium stearate, titanium dioxide, glycerol, macrogol 6000.

The use of Erespal does not replace antibiotic therapy.

For patients with diabetes, the product is prescribed mainly in tablet form, since the syrup contains sucrose.

The drug belongs to the prescription group, so you should consult a doctor before using it.

Before using Erespal, you should consult your doctor.

The description of the drug is based on the officially approved instructions for use.

The anti-inflammatory and anti-bronchoconstrictor activity of the drug Erespal is due to the fact that fenspiride reduces the production of a number of biologically active substances (cytokines, especially tumor necrosis factor (TNF-), arachidonic acid derivatives, free radicals), which play an important role in the development of inflammation and bronchospasm. Inhibition of arachidonic acid metabolism by fenspiride is potentiated by blockade of histamine H1 receptors, because histamine stimulates the formation of its products (prostaglandins and leukotrienes). Fenspiride blocks β-adrenergic receptors, the stimulation of which is accompanied by an increase in the secretion of the bronchial glands. Thus, fenspiride reduces the effect of a number of factors that contribute to the hypersecretion of proinflammatory factors, the development of inflammation and bronchial obstruction. Fenspiride is also characterized by the presence of antispasmodic activity.

Pharmacokinetics of the drug.

Instructions for use of Erespal syrup for children: dosage for children aged 1 year and older, analogues of the drug

Cough is a fairly common problem in children who have recently had an acute respiratory viral infection. It can begin quite harmlessly, but over time develop into a serious complication.

There are a number of medications for the treatment of upper respiratory tract diseases that will help get rid of an annoying cough. Today we will look at an unusual drug to eliminate this symptom called Erespal. When it is shown what type of cough it can handle, as well as side effects - all this can be found in our publication.

Erespal syrup 2 mg/ml 150 ml

Release form, composition of the drug and action

Erespal for children is available in the form of syrup, for adolescents and adults - in tablets. The drug contains an active substance - fenspiride hydrochloride, which has several beneficial properties:

  • anti-inflammatory;
  • antiallergic;
  • antispasmodic;
  • prevents the development of the process of narrowing of the bronchi - bronchoconstriction;
  • reduces the production of bronchial secretions.

1 ml of syrup contains 2 mg of the active substance, and the bottle contains 150 (250) ml of the product. One tablet contains 80 mg of fenspiride hydrochloride. Next, let's look at the auxiliary components included in the syrup. There are quite a lot of them, since fenspiride has a not very pleasant bitter taste and the manufacturer is forced to use components to improve it:

  • licorice root extract;
  • glycerol;
  • orange yellow food coloring S;
  • methylparaben (E-218);
  • para-hydroxybenzoic acid ester;
  • sweeteners;
  • potassium salt of sorbic acid;
  • water;
  • flavoring additive "honey".
  • calcium phosphate disubstituted dihydrate;
  • methyloxypropylcellulose;
  • crospovidone;
  • silica;
  • colloidal silicon dioxide anhydrous;
  • magnesium salt and stearic acid;
  • titanium white;
  • glycerol;
  • macrogol 6000.

The drug begins to act within an hour after administration (this is quite normal for syrup). The tablets begin to work in two hours.

Indications for use of syrup

Erespal is prescribed to children and adults to relieve cough. However, the instructions indicate much more indications. Let's look at them in more detail:

  1. Treatment of inflammation of the ENT organs and respiratory tract caused by infection. These include various types of bronchitis, otitis, laryngitis, tracheitis.
  2. Pulmonary obstruction, pneumonia. In this case, Erespal is prescribed in combination with other drugs.
  3. Allergic rhinitis, which appears seasonally or does not go away throughout the year.
  4. Respiratory symptoms of ARVI, influenza, childhood infectious diseases.
  5. Whooping cough.

What type of cough does the remedy help with?

According to experts, Erespal is effective in combating any type of childhood cough. This distinguishes it from the mass of drugs with similar effects, including cough suppressants, expectorants, and mucolytics. Erespal does not belong to the listed groups, since it has several beneficial properties.

The drug Erespal effectively relieves the symptoms of severe cough and reduces sputum production

The instructions for use indicate that Erespal will relieve the symptoms of allergic cough and rhinitis, reduce the formation of sputum and at the same time expand the airways to facilitate its discharge. That is, the drug will help to quickly cope with dry, wet, as well as lingering cough that plagues the patient for many months.

Due to the listed advantages, the syrup can replace several medications that are prescribed for children when coughing - antiallergic drops, mucolytics, expectorants. Moreover, taking it allows you to reduce the list of prescribed medications, especially when it comes to long-term cough.

Erespal dosage for children

Let's consider the dosage of Erespal for children (see also: use of Prospan syrup for children over one year old). The syrup is indicated for children from 24 months. When prescribing it, the doctor is guided by the child’s body weight at the rate of 4 mcg (2 ml of syrup) per 1 kg of the child’s weight. This is the daily dose of the medicine - it should be divided into the number of doses per day, which can be 2 or 3.

Example: a child weighing 9 kg can receive 18 ml of Erespal syrup per day. This means that parents can give the baby 6 ml of syrup 3 times a day.

For children over 12 years of age and adults, the daily dose is prescribed within ml of syrup. However, in this case, it is advisable to use Erespal tablets, which should be taken two pieces a day - morning and evening. In acute cases of the disease, it is allowed to increase the intake - drink 3 tablets per day.

The course of therapy depends on many factors - the dynamics of the disease, individual sensitivity to the drug, the nature of the cough (dry or productive). The doctor prescribes Erespal’s appointment and he also monitors the effectiveness of the treatment. On average, it is recommended to take the medication for a month or until a stable effect appears.

Erespal can be used in combination with antibiotics and other drugs. Syrup is often prescribed as an adjuvant during the treatment of bronchitis and pneumonia. It is only important to correctly distribute the medications. Erespal should be taken before meals, and antibiotics after.

Contraindications for use

If the patient knows that he has an intolerance to one of the components of the drug, then the drug is not prescribed to him. In addition, the instructions for use indicate other contraindications. The tablets are not given to children under 12 years of age, and the syrup is not recommended:

  • children under 2 years old;
  • people with chronic hypertension, heart failure, cirrhosis of the liver, since the composition contains licorice root;
  • during pregnancy and breastfeeding, since research in this direction has not been conducted.

Possible side effects

According to studies, side effects when taking Erespal do not occur often. In children, undesirable consequences were recorded in 2.4% of cases, in adults - a little more (about 8%). According to statistics, gastrointestinal disorders most often occur as side effects while taking Erespal. Abdominal pain, loose stools, nausea, and vomiting are possible. In addition, it is noted:

  • drowsiness;
  • dizziness;
  • increased heart rate;
  • skin manifestations - urticaria, small rash, redness of the skin, itching;
  • asthenic syndrome, general weakness, severe fatigue.

Erespal is not recommended to be taken with antihistamines, since this drug already has an antiallergic effect. Antihistamines can make you drowsy, and cough syrup will make the problem worse.

Cost of the drug and its analogues

Syrup can be bought in pharmacies at prices ranging from 250 to 600 rubles per bottle. The cost does not depend on the manufacturer (domestic medicines are not cheaper than imported ones), but on the volume. The drug is available in bottles of 150 and 250 ml. A package of tablets (30 pieces) costs about 400 rubles.

Analogs of Erespal containing the same active substance are presented in our table. In addition, we indicated the cost of the drugs and the age at which they can be taken.

When purchasing an analogue, you should take into account the auxiliary components, as well as the concentration of the active substance. There is no significant difference in price between Erespal and its analogues, so the choice should be made based on personal preferences.

The pediatrician prescribed Erespal for my child when we couldn’t get rid of a cough for a long time. It helped a lot - the cough began to “fade away” already on the second day, so we didn’t drink it for long. The disadvantages include the not very pleasant taste.

We tried it once, but didn't try again. The syrup helped the child, the cough quickly went away. But still, I try to use herbal products - they are safer for the child. Well, all sorts of rubbing, compresses, baths.
