Description of the active substance Tetrizoline/Tetryzoline.
Formula: C13H16N2, chemical name: 4,5-Dihydro-2-(1,2,3,4-tetrahydro-1-naphthalenyl)-1H-imidazole (as hydrochloride).
Pharmacological group: vegetotropic agents/adrenomimetic agents/Alpha-adrenomimetics; organotropic drugs/ophthalmic drugs; organotropic agents/respiratory agents/anticongestants.
Pharmacological action: anticongestive, vasoconstrictor.
Pharmacological properties
Tetrizoline has a stimulating effect on alpha-adrenergic receptors. When used topically in the form of eye drops, it reduces swelling of the conjunctiva, sensations of irritation, burning, soreness, itching of the mucous membrane of the eyes, and reduces lacrimation. When applied topically in the form of nasal drops, it narrows the small arterioles of the nasal passages, reduces secretion and reduces swelling of the nasal mucosa. The effects of tetrizoline develop within a few minutes and last 4 – 8 hours. When used topically, tetrizoline is almost not absorbed.
Indications
Nasal drops: swelling of the nasal mucosa in people who suffer from rhinitis, sinusitis, pharyngitis, hay fever; if it is necessary to carry out therapeutic and diagnostic measures to reduce swelling of the nasal mucosa.
Eye drops: swelling of the conjunctiva, irritation of the conjunctiva (when exposed to physical and chemical factors, including dust, smoke, chlorinated water, cosmetics, bright light, contact lenses), secondary hyperemia in allergic eye diseases, catarrhal conjunctivitis.
Method of administration of tetrizoline and dose
Intranasally, patients over 6 years old - 0.1% solution, 2-4 drops, children 2-6 years old - 0.05% solution, 2-3 drops in each nasal passage with an interval of at least 3 hours. Tetrizoline should not be used for more than 3 to 5 days (except on doctor's recommendations). Re-use is possible only after a break of several days.
Conjunctivally, 2 - 3 times a day, 1 - 2 drops in each eye in the lower conjunctival sac. Do not use continuously for more than 4 days without consulting an ophthalmologist. If there is no improvement within 2 days, then therapy should be discontinued.
The dosage regimen should be strictly observed. Overdose and prolonged use should be avoided. With prolonged use there is a risk of developing toxic effects. The possibility of overdose, which is caused by systemic absorption of tetrizoline, is increased in young children (especially if accidentally swallowed). Tetrizoline eye drops should only be used for mild eye irritation. If hyperemia and irritation of the conjunctiva are associated with an infectious process, chemical injury to the cornea, or the presence of a foreign body, then consultation with an ophthalmologist is necessary. If within 2 days after starting to use eye drops, the symptoms of the disease become more pronounced, persist, or an adverse reaction develops, then it is necessary to discontinue the drug and consult an ophthalmologist. If you develop a headache, intense pain in the eyes, blurred vision, redness of the eyes, sudden appearance of floating spots in front of the eyes, double vision or pain when exposed to light, you should immediately consult a doctor. Contact lenses are removed before instillation of the drug and reinstalled 15 minutes after it. Due to the possible deterioration of the clarity of soft contact lenses, direct contact of eye drops with them should be avoided. During therapy, care should be taken when working with machinery and driving a car (after using eye drops, blurred vision and pupil dilation may occur).
Contraindications for use
Hypersensitivity; nasal drops: up to 2 years of age (for a 0.05% solution), up to 6 years (for a 0.1% solution), dry rhinitis; eye drops: age up to 2 years, severe eye pathology (dry eye syndrome, glaucoma, epithelial endothelial dystrophy of the cornea).
Restrictions on use
Severe pathology of the circulatory system (including arrhythmia, coronary heart disease, arterial hypertension, cardiac aneurysm), diabetes mellitus, hyperthyroidism, pheochromocytoma, period of therapy with drugs that increase blood pressure, including monoamine oxidase inhibitors.
Use during pregnancy and breastfeeding
Due to the risk of systemic adverse reactions, the use of tetrizoline during pregnancy and breastfeeding is possible only after careful comparison of the expected benefit to the mother and the possible risk to the child or fetus. It is not known whether tetrizoline is excreted in breast milk. There are no reproductive studies in animals.

Interaction of tetrizoline with other substances
Tetrizoline is incompatible with monoamine oxidase inhibitors (risk of developing hypertension).
Overdose
In case of an overdose of tetrizoline, pupil dilation, cyanosis, nausea, fever, tachycardia/bradycardia, convulsions, arterial hypertension, shock-like syndrome with bradycardia and hypotension, and dizziness develop. In children, an overdose can manifest itself as depression of the central nervous system, which is accompanied by excessive sweating, drowsiness, severe hypotension, even shock. Excipients: benzalkonium chloride, sorbitol non-crystallizing solution, povidone, disodium edetate, sodium citrate, benzyl alcohol, concentrated hydrochloric acid, purified water.
A nasal drug with a predominant decongestant and vasoconstrictor effect.
Pharmacological properties:
Pharmacodynamics. Tetrahydrozoline hydrochloride belongs to the group of sympathomimetics and has a direct stimulating effect on alpha-adrenergic receptors of the sympathetic nervous system. However, it has no or almost no effect on beta-adrenergic receptors. As a sympathomimetic amine, the drug has a vasoconstrictor and decongestant effect when it comes into contact with the nasal mucosa, which leads to a narrowing of the small arterioles of the nasal passages, a decrease in swelling of the nasal mucosa and a decrease in secretion.
Pharmacokinetics. After topical application, the effect develops within 1 minute and lasts 4-8 hours. Indications for use:

Directions for use and dosage:
Tetrizoline, nasal drops 1.0 mg/1 ml. If there are no other prescriptions, then adults and children over 6 years of age are prescribed 2-4 drops of the drug in each nostril 3-5 times a day.
Tetrizoline, nasal drops 0.5 mg/1 ml. If there are no other prescriptions, children aged 2 to 6 years are prescribed 2-3 drops of the drug in each nostril 3-4 times a day.
Tetrizoline should be used no more than once every 4 hours.
As a rule, less frequent use of the drug is sufficient, since in most cases the drug has a long-lasting effect (often up to 8 hours). A single use of the drug before bedtime relieves nasal congestion throughout the night, but may cause insomnia due to the effect of tetrizoline on the central nervous system.
Tetrizoline can be used for a short time. The duration of treatment should be no more than 5 days for adults and no more than 3 days for children. The drug can be re-prescribed after a few days.
Features of application:
In patients with severe cardiovascular diseases (for example, coronary heart disease (CHD), arterial hypertension, arrhythmia, aneurysm, pheochromocytoma) and metabolic disorders (hyperthyroidism, diabetes), in patients taking MAO inhibitors or other drugs that increase blood pressure, Tetrizoline can be used only after careful comparison of the possible benefits and risks for the patient.

Long-term use of imidazole derivatives can have a damaging effect on the mucosa, suppressing the activity of the ciliated epithelium. Irreversible damage to the mucosa can lead to the development of dry rhinitis.
Using nasal drops at night may cause insomnia because tetrizoline affects the central nervous system.
Use during pregnancy and breastfeeding. Since there is a possibility of systemic side effects (mainly vasoconstrictor effects), the use of the drug during pregnancy and lactation is possible only after careful comparison of the possible benefits to the mother and the risk to the fetus (child).
Impact on the ability to drive a car and other moving mechanisms. After long-term use of the drug in dosages exceeding the recommended ones, a general effect on the cardiovascular system and central nervous system cannot be excluded. Contraindications:
- hypersensitivity to the components of the drug;
- children under 2 years of age (tetrizoline solution 0.5 mg/1 ml);
- children under 6 years of age (tetrizoline solution 1.0 mg/1 ml).
Overdose:
Symptoms of overdose are mydriasis, nausea, cyanosis, fever, muscle twitching, tachycardia, arrhythmia, asystole, arterial hypertension, pulmonary edema, respiratory dysfunction, and mental disorders.
In some cases, excessive absorption of imidazole derivatives by α-sympathomimetics can cause depression of the central nervous system, which manifests itself in the form of drowsiness, decreased body temperature, bradycardia, shock-like hypotension, apnea and coma.
There is no specific antidote. The use of stimulant drugs is contraindicated. Although the symptoms of an overdose are quite threatening, they usually go away easily and the patient recovers quickly.
Therapeutic measures for overdose. If necessary, activated charcoal is used, gastric lavage is performed, oxygen therapy is performed, fever is treated, and anticonvulsants are used. To lower blood pressure, 5 mg of phentolamine is administered slowly intravenously in an isotonic saline solution, or 100 mg is prescribed orally. In case of hypotension, vasoconstrictor drugs are contraindicated. If anticholinergic symptoms develop, physostigmine should be used as an antidote.

Tetrizoline hydrochloride is easily soluble in water and alcohol, very slightly soluble in chloroform, practically insoluble in ether. The pH of a 1% aqueous solution is 5.0–6.5. Molecular weight 236.75.
Pharmacology
Stimulates alpha adrenergic receptors. When applied topically in the form of eye drops, it reduces swelling of the conjunctiva, burning sensation, irritation, itching, soreness of the mucous membrane of the eyes, and lacrimation.
When used in the form of nasal drops, it causes a narrowing of the small arterioles of the nasal passages, reduces swelling of the nasal mucosa and reduces secretion.
When applied topically, it is practically not absorbed. The effect develops within a few minutes and lasts 4–8 hours.
Application of the substance Tetrizoline
Swelling of the conjunctiva, secondary hyperemia in allergic eye diseases, irritation of the conjunctiva (as a result of exposure to chemical and physical factors, including smoke, dust, chlorinated water, bright light, cosmetics, contact lenses), catarrhal conjunctivitis.
Swelling of the nasal mucosa in patients suffering from rhinitis, pharyngitis, sinusitis, hay fever, as well as to reduce swelling of the nasal mucosa if diagnostic and therapeutic measures are necessary.
Contraindications
Hypersensitivity; eye drops: severe eye diseases (glaucoma, dry eye syndrome, epithelial endothelial corneal dystrophy), children under 2 years of age; nasal drops: dry rhinitis, children up to 2 years of age (0.05% solution), up to 6 years of age (0.1% solution).
Restrictions on use
Severe cardiovascular diseases (including ischemic heart disease, arrhythmia, cardiac aneurysm, arterial hypertension), hyperthyroidism, diabetes mellitus, pheochromocytoma, period of treatment with drugs that increase blood pressure, incl. MAO inhibitors.
Use during pregnancy and breastfeeding
Due to the likelihood of systemic side effects, use during pregnancy and breastfeeding is possible only after careful comparison of the possible benefits to the mother and the risk to the fetus or child. No reproductive studies have been conducted in animals. It is not known whether tetrizoline is excreted into breast milk.
Side effects of the substance Tetrizoline
Eye drops: pupil dilation, increased intraocular pressure, burning sensation and reactive hyperemia of the eye.
Nasal drops: burning and tingling in the nose, sneezing, dry nasal mucosa; with prolonged use or exceeding the dose - secondary edema, hyperemia of the nasal mucosa.
Systemic effects: headache, dizziness, weakness, tremor, insomnia, increased blood pressure, palpitations, nausea, hyperglycemia, allergic reactions.
Interaction
Incompatible with MAO inhibitors (possible development of hypertension).
Overdose
Symptoms: pupil dilation, nausea, cyanosis, fever, convulsions, tachycardia/bradycardia, arterial hypertension, dizziness, shock-like syndrome with hypotension and bradycardia. Overdose in children can manifest as depression of the central nervous system, accompanied by drowsiness, excessive sweating, severe hypotension, even shock.
Treatment: administration of activated carbon, gastric lavage, symptomatic therapy (oxygen inhalation, use of antipyretics, anticonvulsants). A specific antidote is unknown.

The dosage regimen must be strictly observed. Long-term use and overdose should be avoided. With prolonged use, toxic effects may develop. The risk of overdose due to systemic absorption of the drug is high in newborns and small children (especially if accidentally swallowed).
Tetrizoline in the form of eye drops is advisable to use only for mild eye irritation. If irritation and hyperemia of the conjunctiva are associated with an infectious disease, the presence of a foreign body or chemical injury to the cornea, consultation with an ophthalmologist is necessary.
If within 48 hours after starting the use of eye drops, the symptoms of the disease persist/become more pronounced or a side effect appears, you should stop using the drug and consult an ophthalmologist. If you experience intense eye pain, headache, blurred vision, sudden appearance of floaters in front of your eyes, red eyes, pain when exposed to light, or double vision, you should immediately consult a doctor.
When wearing contact lenses, they are removed before instillation of the drug and reinstalled 15 minutes after it. Direct contact of eye drops with soft contact lenses should be avoided due to possible disruption of their transparency.
During the treatment period, care must be taken when driving a car and operating machinery (after using eye drops, the pupil may dilate and blurred vision may occur).

Tetrizoline
Runny nose. Swelling of the mucous membrane of the nasal cavity and nasopharynx. Minor eye irritation; itching, swelling and hyperemia (redness) of the conjunctiva (outer shell of the eyes), burning, lacrimation, injection (redness) of the sclera, caused by chemical and physical factors (dust, cosmetics, swimming in chlorinated water, exposure to bright light); allergic conjunctivitis (inflammation of the outer membrane of the eyes).
Sympathomimetic. When used topically, it causes a vasoconstrictor effect that develops within a few minutes and persists for a long time.
For instillation into the nose, a single dose for adults and children over 6 years old is 2-4 drops in each nasal passage; Children aged 2 to 6 years are prescribed a 0.5% solution of drops, a single drop in each nasal passage. Nasal drops should be used at intervals of at least 3 hours.
Eye drops into the lower conjunctival sac (the cavity between the back surface of the lower eyelid and the front surface of the eyeball) of each eye 2-3 times a day.
Continuous use of eye drops for more than 4 days is not recommended. If signs of eye irritation persist after 48 hours, stop using the product and consult a doctor.
Increased sensitivity to the product. For the use of eye drops, glaucoma (increased intraocular pressure), epidermal endothelial dystrophy (thinning) of the cornea (the transparent membrane of the eye), simultaneous use of MAO inhibitors. Children under 3 years of age are not prescribed the product in the form of eye drops. Children under 6 years of age are contraindicated in the use of 0.1% nasal drops.
The product is prescribed with caution for arterial hypertension (high blood pressure), hyperthyroidism (overactive thyroid syndrome), heart disease, diabetes, during treatment with antidepressants, during pregnancy, and breastfeeding.
Dilated pupils, increased intraocular pressure, reactive hyperemia (redness), headache, nausea. Systemic effects are possible - arterial hypertension (increased blood pressure), cardiac dysfunction, hyperglycemia (increased blood sugar levels). Allergic reactions are possible. When using nasal drops, you may experience a burning and tingling sensation in the nose, sneezing, dry nasal cavity, headache, drowsiness, weakness, tremor (trembling of the limbs), dizziness, sleep disturbance, palpitations. With prolonged use or exceeding the dose, secondary edema of the nasal mucosa may develop.
Eye drops in a bottle of 8 ml, 15 ml and in a dropper bottle of 10 ml (1 ml of 500 mcg of tetrizoline hydrochloride). Drops for intranasal (into the nasal cavity) use in a bottle of 15 ml (1 ml - 1 g of tetrizoline), drops for intranasal use for babies in a bottle of 15 ml (1 mlmg of tetrizoline).

Tetrizoline: instructions for use
Compound
1 ml of the drug contains 0.5 mg or 1.0 mg of tetrahydrozoline hydrochloride as an active ingredient.
Excipients: benzalkonium chloride, sorbitol non-crystallizing solution, povidone, disodium edetate, sodium citrate, benzyl alcohol, concentrated hydrochloric acid, purified water.
Description
Indications for use
- symptomatic treatment of rhinitis, pharyngitis, sinusitis, hay fever;
— to reduce swelling of the nasal mucosa before diagnostic procedures.
Contraindications
- hypersensitivity to the components of the drug;
— chronic atrophic rhinitis;
- glaucoma (especially angle-closure);
- children under 2 years of age (tetrizoline solution 0.5 mg/1 ml);
- children under 6 years of age (tetrizoline solution 1.0 mg/1 ml).
Pregnancy and lactation
Since there is a possibility of systemic side effects (mainly vasoconstrictor effects), the use of the drug during pregnancy and lactation is possible only after careful comparison of the possible benefits to the mother and the risk to the fetus (child).
Directions for use and doses
Tetrizoline, nasal drops 1.0 mg/1 ml
If there are no other prescriptions, then adults and children over 6 years of age are prescribed 2-4 drops of the drug in each nostril 3-5 times a day.
Tetrizoline, nasal drops 0.5 mg/1 ml
If there are no other prescriptions, children aged 2 to 6 years are prescribed 2-3 drops of the drug in each nostril 3-4 times a day.
Tetrizoline should be used no more than once every 4 hours.
As a rule, less frequent use of the drug is sufficient, since in most cases the drug has a long-lasting effect (often up to 8 hours). A single use of the drug before bedtime relieves nasal congestion throughout the night, but may cause insomnia due to the effect of tetrizoline on the central nervous system.
Tetrizoline can be used for a short time. The duration of treatment should be no more than 5 days for adults and no more than 3 days for children. The drug can be re-prescribed after a few days.
Side effect
From the organ of vision: dilated pupils, increased intraocular pressure, reactive hyperemia.
From the central nervous system: headache; when using drops in the nose - drowsiness, weakness, tremor, dizziness, sleep disturbance (insomnia).
From the digestive system: nausea.
From the cardiovascular system: arterial hypertension, cardiac dysfunction, tachycardia.
From the endocrine system: hyperglycemia, sweating.

Overdose
Symptoms of overdose are mydriasis, nausea, cyanosis, fever, muscle twitching, tachycardia, arrhythmia, asystole, arterial hypertension, pulmonary edema, respiratory dysfunction, and mental disorders.
In some cases, excessive absorption of imidazole derivatives by α-sympathomimetics can cause depression of the central nervous system, which manifests itself in the form of drowsiness, decreased body temperature, bradycardia, shock-like hypotension, apnea and coma.
There is no specific antidote. The use of stimulant drugs is contraindicated. Although the symptoms of an overdose are quite threatening, they usually go away easily and the patient recovers quickly.
Therapeutic measures for overdose. If necessary, activated charcoal is used, gastric lavage is performed, oxygen therapy is performed, fever is treated, and anticonvulsants are used. To lower blood pressure, 5 mg of phentolamine is administered slowly intravenously in an isotonic saline solution, or 100 mg is prescribed orally. In case of hypotension, vasoconstrictor drugs are contraindicated. If anticholinergic symptoms develop, physostigmine should be used as an antidote.
Interaction with other drugs
With the simultaneous administration of MAO blockers and tricyclic antidepressants, an increase in blood pressure is possible. Co-administration of other vasoconstrictor drugs increases the risk of side effects.

Long-term use of imidazole derivatives can have a damaging effect on the mucosa, suppressing the activity of the ciliated epithelium. Irreversible damage to the mucosa can lead to the development of dry rhinitis.
Using nasal drops at night may cause insomnia because tetrizoline affects the central nervous system.
