Over the past decades, the pharmacotherapy of allergic diseases has been replenished with a large group of H1 receptor antagonists , which, while exhibiting a pronounced antihistamine effect, are distinguished by their pharmacokinetic and pharmacodynamic properties.

Despite the emergence of second-generation antihistamines, first-generation drugs will continue to remain in the arsenal of drugs for widespread use in clinical allergology due to:

  • with the constant acquisition of new clinical data based on long-term experience in the use of these drugs and the possibility of a differentiated approach to their prescription;
  • with the presence of first generation H1 antagonists of some pharmacological effects (such as antiserotonin activity, sedative effect, anticholinergic effect, etc.).

Suprastin is a classic first-generation antihistamine with pronounced antihistamine and M-anticholinergic effects.

In recent years, new data have emerged on the mechanisms of action of Suprastin, thanks to which it has found wide use in the practice of doctors of various specialties.

Thus, in 2003, in the April issue of the journal J Allergy Clin Immunol, a meta-analysis of comparative studies examining the sedative effects of second- and first-generation H1 blockers was published . According to the authors of this publication, the difference in the frequency of sedation between first and second generation antihistamines is not as significant as previously thought [1].

Data from 18 clinical trials showed that study results varied widely, with the level of sedation in some studies being lower with first-generation antihistamines than in the placebo group or another group using second-generation antihistamines.

New data have emerged on the pharmacokinetics of first-generation antihistamines: in particular, it has become known that this indicator for chlorpheniramine, brompheniramine and hydroxyzine in adults exceeds 14-20 hours, so they can be used once or twice a day without a decrease in effectiveness and with a decrease in frequency side effects [2].

Suprastin is a representative of classical first-generation antihistamines and is prescribed for allergic diseases such as urticaria, serum sickness, angioedema, hay fever, allergic rhinopathies, allergic conjunctivitis, contact dermatitis, acute and chronic eczema, drug rashes, itchy dermatoses. , itching from insect bites.

Nevertheless, publications have appeared on the effectiveness of Suprastin in other pathologies: for example, in people suffering from allergies, or in conditions where the prescription of this drug is pathogenetically justified.

Use of Suprastin for ARVI

Unlike second-generation H1 - blockers and first-generation drugs that do not have anticholinergic activity (Diazolin and Fenkarol), the mechanism of action of Suprastin is associated both with its ability to block H1 - histamine receptors and with competitive antagonism towards muscarinic receptors, which mediate parasympathetic stimulation of nasal gland secretion and vasodilation. In addition, due to its ability to penetrate the blood-brain barrier, Suprastin affects the receptor formations of the medulla oblongata and hypothalamus and affects the sneezing center, blocking the parasympathetic cascade in relation to the nasal glands and blood vessels. Clinically, Suprastin for ARVI effectively reduces symptoms of rhinitis such as swelling of the mucous membrane and nasal congestion, rhinorrhea, itching and sneezing [3].

Prescribing Suprastin twice a day at a daily dose of 50 mg (25 mg per dose) for five days already on the second day of administration reduces nasal congestion, discharge, and sneezing. On the fourth or fifth day of treatment with Suprastin, nasal breathing is practically restored, nasal discharge, sneezing and itching in the nose stop (see Photo 1). An analysis of the dynamics of the disease in groups comparable in gender, age and nature of the pathological process showed that in patients receiving Suprastin, rhinitis symptoms regressed 1.6–2.3 times faster compared to the group of patients receiving vitamins and herbal preparations. As the manifestations of rhinitis are relieved, the patient’s general condition improves, the intensity of the sore throat decreases, and the severity of the cough syndrome caused by the flow of discharge from the nasal cavity into the larynx and trachea decreases (see Figure 1). In addition, the use of Suprastin allows, in certain cases, to avoid the use of nasal vasoconstrictor drugs, which, with prolonged use, can cause drug-induced rhinitis. Patients already using nasal decongestants can significantly reduce the frequency of their use (by 40%) [4].

Use of Suprastin for asthma

The question of the possible use of antihistamines for bronchial asthma (BA) arose soon after their introduction into clinical practice (1940–1950): they began to be used for any allergic processes, including BA. However, soon works appeared that not only indicated the lack of clinical effect of antihistamines in asthma, but also caused a deterioration in the condition of patients, making it difficult to separate sputum (1950–1955). Studies conducted in 1980–90 (Schulle et al.) showed that the clinical significance of the antiasmatic effect of antihistamines is not great, however, information about side effects was not confirmed. It has been suggested that antihistamines for bronchial asthma are prescribed in cases where there are symptoms of other concomitant allergic diseases.

Today, the position of the American Academy of Allergy and Immunology regarding the use of antihistamines in the treatment of bronchial asthma is as follows: although not first-line drugs, the drugs are nevertheless not contraindicated in patients with bronchial asthma with concomitant allergic diseases (for example, allergic rhinitis). In recent years, it has become known that many antihistamines, including those with dual (anticholinergic and antihistamine) effects, when used in patients with bronchial asthma, have a positive effect due to their bronchodilator properties [5].

In addition, bronchial asthma in children under five years of age is accompanied by hypersecretion of sputum associated with the physiological characteristics of early age. In such a situation, many practitioners recommend using Suprastin to reduce sputum production.

Use of Suprastin for the prevention of acute subglottic laryngitis (“false croup”) in children with acute respiratory infections

Suprastin as an antihistamine is prescribed for acute respiratory viral infections, since the allergic component plays an important role in the mechanism of development of edema of the laryngeal mucosa, especially in “false croup”. In addition, Suprastin has an M-cholinolytic effect, due to which it also eliminates swelling of the mucous membrane. It is significant for pediatric practice that Suprastin is approved for use in children starting from the first month of life.

Unspecified and cholinergic urticaria

With cholinergic urticaria, there is an increased density of cholinergic receptors in the skin. For cholinergic and unspecified urticaria, Suprastin is the drug of choice compared to second-generation drugs [7].

Atopic dermatitis and other itchy allergic dermatoses (eczema, chronic recurrent urticaria, etc.)

The painful itching of the skin that occurs with these diseases significantly reduces the quality of life, often causing insomnia and the development of neurotic disorders. In such cases, the sedative effect of first-generation antihistamines is useful. When treating patients with this pathology, Suprastin can be combined with second-generation antihistamines, for example with Erolin (one Erolin tablet in the morning, one Suprastin tablet in the evening) [7].

A leading American dermatologist, professor at Harvard University T. Fitzpatrick, also recommends, in case of severe itching, to give preference to antihistamines with a sedative effect [8].

According to French allergist Alain Didier, for the treatment of allergies, not only antihistamine, but also anticholinergic action is necessary. “The release of acetylcholine and substance P from presynaptic nerve terminals may support the allergic inflammatory response at the level of the cholinergic nervous system in the late phase of the anaphylactic reaction. The anticholinergic effect blocks the late phase of the anaphylactic reaction (absent in selective antihistamines, such as cetirizine) [9].

Premedication before diagnostic (radiocontrast and endoscopic) examinations and surgical interventions

In these cases, the use of Suprastin is based on its ability to enhance the effect of analgesics and have an antiemetic effect. The administration of Suprastin is indicated before X-ray contrast studies, when the patient is given infusions of high molecular weight iodine-containing agents (verografin, urografin, isopak, etc.), which cause the development of adverse reactions in 3-14% of patients. The latter are manifested by itching and hyperemia of the skin, urticaria, Quincke's edema, laryngeal edema, bronchospasm and anaphylactoid shock. They are caused by the release of histamine from mast cells and basophils, as well as activation of the complement system. Side effects are more often recorded in patients with allergic and cardiovascular diseases, including taking b-blockers [7].

Use for ENT diseases

In acute otitis media, Suprastin reduces swelling of the mucous membrane of the tympanic cavity and auditory tube. Thanks to its anticholinergic effect, Suprastin improves the aeration of the tympanic cavity, restoring the function of the auditory tube.

For sinusitis, Suprastin improves the drainage function of natural anastomosis, reducing swelling of the mucous membrane of the nasal cavity and paranasal sinuses (due to the M-anticholinergic mechanism of action), and reduces inflammation (due to the antihistamine mechanism of action).

Acute allergic reactions (urticaria, angioedema, anaphylactic shock)

In cases of acute allergic reactions, when parenteral administration of antihistamines is necessary, the presence of the injection form of Suprastin determines its choice as a first-line drug. In case of anaphylactic shock after the administration of adrenaline, Suprastin is administered intravenously slowly (1-2 ml of 2% solution), a prerequisite is to control the level of systolic blood pressure, which should not be lower than 90 mmR.
