Suprastin is an antiallergic drug, a blocker of histamine H1 receptors.
Release form and composition
- solution for intravenous (i.v.) and intramuscular (i.m.) administration: clear, colorless liquid with a slight specific odor (1 ml in ampoules with a dark red code ring and a break point; in a blister pack, 5 ampoules each; in a cardboard box 1 or 2 packages);
- tablets: disc-shaped, chamfered, SUPRASTIN engraved on one side and a line on the other side; color from grayish-white to white, practically odorless (10 pcs. in blisters, 2 blisters in a cardboard pack; 20 pcs. in dark glass bottles, 1 bottle in a cardboard pack).
1 ampoule with solution contains:
- active substance: chloropyramine hydrochloride – 20 mg;
- auxiliary components: water for injection.
1 tablet contains:
- active substance: chloropyramine hydrochloride – 25 mg;
- auxiliary components: gelatin, stearic acid, sodium carboxymethyl starch (type A), potato starch, talc, lactose monohydrate.
Pharmacological properties
Pharmacodynamics
Chloropyramine hydrochloride is a chlorinated analogue of tripelenamine (pyribenzamine), a classic antihistamine from the group of ethylenediamine antihistamines. It has an antihistamine and m-anticholinergic effect, has an antiemetic, moderate antispasmodic and peripheral anticholinergic effect.
Pharmacokinetics
- absorption: chloropyramine hydrochloride after oral administration is almost completely absorbed from the gastrointestinal tract (GIT);
- distribution and metabolism: the substance is well distributed throughout systems and organs, including the central nervous system (CNS) and is intensively metabolized through the liver enzyme system;
- excretion: Suprastin is excreted mainly by the kidneys; In children, the rate of drug elimination is higher than in adults.
Indications for use
- serum sickness;
- food and drug allergies;
- hives;
- conjunctivitis;
- seasonal and year-round allergic rhinitis;
- contact dermatitis;
- eczema in acute and chronic course;

Contraindications
- acute attacks of bronchial asthma;
- lactase deficiency, lactose intolerance, glucose-galactose malabsorption syndrome (tablets, because 1 piece contains 116 mg of lactose);
- period of pregnancy and lactation (breastfeeding);
- age up to 3 years (tablets);
- neonatal period (full-term and premature babies);
- hypersensitivity to any of the components of the drug.
Relative contraindications (requiring caution): angle-closure glaucoma, urinary retention and prostatic hyperplasia, liver/kidney dysfunction, cardiovascular disease, old age.
Instructions for use of Suprastin: method and dosage
Pills
Suprastin tablets are taken orally at the same time as food, not chewed and washed down with a sufficient amount of water.
- children aged 3–6 years: ½ pcs. (12.5 mg) 2 times a day;
- children aged 6–14 years: ½ pcs. (12.5 mg) 2–3 times a day;
- children over 14 years old and adult patients: 1 pc. (25 mg) 3–4 times a day; daily dose – 75–100 mg.
If the patient has no side effects, the dose can be gradually increased, but the maximum daily dose should not exceed 2 mg/kg body weight.
The course of therapy depends on the symptoms of the disease, its duration and clinical course.
Solution for IM and IV administration
Suprastin in the form of a solution is administered intravenously and intramuscularly. The drug is administered into a vein only in emergency cases, always under the supervision of a specialist.
Recommended initial doses of Suprastin for children:
- 1–12 months: ¼ ampoule (0.25 ml) IM;
- 1–6 years: ½ ampoule (0.5 ml) IM;
- 6–14 years: ½–1 ampoule (0.5–1 ml) IM.
For adult patients, the drug is recommended to be administered intravenously, in a daily dose of 1–2 ampoules (1–2 ml). In case of severe allergy, treatment should begin with careful, slow intravenous administration of the drug, continuing therapy with intramuscular injections or taking tablets.
The dose may be increased under the control of observed side effects and patient response, but not more than 2 mg/kg body weight.
Side effects
Side effects are usually extremely rare and go away on their own after discontinuation of the drug:
- hematopoietic system: very rarely - agranulocytosis, leukopenia, hemolytic anemia, other changes in the cellular composition of the blood;
- central and peripheral nervous system: fatigue, drowsiness, dizziness, tremor, nervous excitement, euphoria, headache, convulsions, encephalopathy;
- organ of vision: blurred vision, increased intraocular pressure, glaucoma;
- cardiovascular system: tachycardia, decreased blood pressure (BP), arrhythmia (the direct connection of these side effects with taking the drug is not always proven);
- digestive system: dry mouth, abdominal discomfort, nausea/vomiting, constipation, diarrhea, loss or increased appetite, epigastric pain;
- urinary system: urinary retention, difficulty urinating;
- musculoskeletal system: muscle weakness;
- other reactions: photosensitivity, hypersensitivity.
If any of the above effects occur, or other negative reactions occur, you should stop taking the drug and immediately consult a doctor.
Overdose
Symptoms of an overdose of chloropyramine hydrochloride are: anxiety, incoordination, convulsions, ataxia, athetosis, hallucinations. In young children: dry mouth, anxiety, agitation, fixed dilated pupils, sinus tachycardia, facial flushing, fever, urinary retention, coma. In adults: facial flushing and fever are observed sporadically, followed by a period of excitement followed by convulsions, post-convulsant depression and coma.
To treat the condition (when taking the drug for a period of no more than 12 hours), gastric lavage is performed, taking into account the anticholinergic effect and antiemetic effect of Suprastin. Activated carbon is prescribed, breathing and blood pressure parameters are monitored, and symptomatic therapy is carried out. If necessary, resuscitation measures are carried out. There is currently no data on a specific antidote for chloropyramine.

Taking Suprastin in combination with ototoxic drugs can mask early signs of their negative effect on hearing and the functioning of the vestibular apparatus.
Patients with liver and kidney diseases may require a downward dose adjustment of the drug; therefore, the presence of renal/hepatic insufficiency should be reported to the doctor.
When taking the drug at night, the symptoms of reflux esophagitis may increase.
During therapy with Suprastin, you should refrain from drinking alcoholic beverages, since chloropyramine may enhance the effect of ethanol on the central nervous system.
Long-term use of antihistamines can cause disorders of the hematopoietic system such as agranulocytosis, hemolytic anemia, leukopenia, thrombocytopenia. In case of an unexplained increase in temperature in a patient during long-term use of Suprastin, observation of pallor of the skin, jaundice, laryngitis, formation of ulcers in the mouth, the appearance of hematomas, unusual and long-lasting bleeding, it is necessary to conduct an extended clinical blood test to determine the number of formed elements. If the blood formula changes, according to test results, stop taking the drug.
Impact on the ability to drive vehicles and complex mechanisms
Suprastin, especially at the beginning of the course of treatment, can cause increased fatigue, drowsiness, dizziness, and therefore at this time it is prohibited to perform all types of work associated with an increased risk of accidents, including driving vehicles. The duration of the initial period and the degree of restriction on working with complex machines and mechanisms are determined by the doctor individually for each patient.
Use during pregnancy and lactation
Due to the lack of adequate clinical studies of the use of antihistamines during pregnancy, the use of Suprastin by pregnant women, especially in the first trimester and the last month, is possible only if the potential benefit to the mother exceeds the possible risk to the fetus.
If the drug must be used during lactation, breastfeeding should be suspended.
Use in childhood
Suprastin solution is contraindicated for use in children (including premature infants) during the neonatal period, from birth and up to 28 days.
The drug in tablet form is prohibited from giving Suprastin to children under 3 years of age.
For impaired renal function
The drug is used with caution in case of impaired renal function. It may be necessary to change the dosage regimen and reduce the dose, since chloropyramine is excreted mainly by the kidneys.
For liver dysfunction

Instructions for use
- For a baby under one year old, take 1/4 tablet of suprastin, place it between two spoons and crush until a powder forms. Add liquid food to the resulting powder, then give the child the medicine from a syringe without a needle or from a spoon, holding the baby upright.
- For a child older than one year, divide the suprastin tablet into halves or quarters, give the baby the required dose and a glass of water, asking him to swallow the drug and wash it down with water. If the child is unable to swallow the medicine, it can be crushed as for a child under one year old.
- It is advisable for children to be given medication injections by a healthcare professional. The required dose is taken with a syringe and injected into the child’s buttocks.
Special precautions
- To store the medicine, choose a place where children cannot find it. The optimal storage temperature is +15+25°C.
- Do not use suprastin, which has expired (it is 5 years and is indicated on the drug packaging).
- An overdose of the drug is manifested by anxiety, excitement of the nervous system, redness of the face, tachycardia, dilated pupils, fever and other symptoms. If you suspect that the dose has been exceeded, you should rinse the stomach, give the baby activated charcoal and at the same time call an ambulance.
- Suprastin can enhance the effect of sedatives, sympatholytics, antidepressants, atropine and tranquilizers, so their combined use should be careful.

- Since the tablets of this medicine contain lactose, it is important to take this into account if a child has problems with carbohydrate metabolism (lactose intolerance, lactase deficiency, galactose or glucose malassorption).

Pharmacokinetics

The therapeutic effect of chloropyramine develops within 15–30 minutes after oral administration, reaches a maximum within the first hour after administration and lasts at least 3–6 hours. In children, drug elimination occurs faster than in adult patients.

Use during pregnancy and breastfeeding
There have been no adequate, well-controlled studies of the use of antihistamines in pregnant women. In accordance with this, Suprastin ® should be taken during pregnancy (especially in the first trimester and the last month) only if the potential benefit to the mother outweighs the possible risk to the fetus.
If it is necessary to use the drug during lactation, the issue of stopping breastfeeding should be decided.
Side effects
Side effects, as a rule, occur extremely rarely, are temporary, and disappear after discontinuation of the drug.
From the blood and lymphatic system: very rarely - leukopenia, agranulocytosis, hemolytic anemia and other changes in the cellular composition of the blood.
From the central nervous system: drowsiness, fatigue, dizziness, nervous agitation, tremor, headache, euphoria, convulsions, encephalopathy.
From the organ of vision: blurred vision, glaucoma, increased intraocular pressure.
From the cardiovascular system: decreased blood pressure, tachycardia, arrhythmia. A direct connection between these side effects and the use of the drug has not always been established.

Orally, during meals, without chewing and with plenty of water.
Adults: prescribe 1 tablet. 3–4 times a day (75–100 mg/day).
Children: aged 3 to 6 years - 1/2 table. 2 times a day; from 6 to 14 years - 1/2 table. (12.5 mg) 2-3 times a day.
The dose can be gradually increased if the patient has no side effects, but the maximum dose should never exceed 2 mg/kg.
The duration of treatment depends on the symptoms of the disease, its duration and course.

Adults: the recommended daily dose is 1–2 ml (1–2 amp.) IM.
Children: recommended initial doses - from 1 to 12 months - 0.25 ml (1/4 amp.) IM; from 1 to 6 years - 0.5 ml (1/2 amp.) IM; from 6 to 14 years - 0.5–1 ml (1/2–1 amp.) IM.
The dose may be increased cautiously depending on the patient's response and observed side effects. However, the dose should never exceed 2 mg/kg. In case of severe allergies, treatment should begin with a careful, slow intravenous injection, and then continue with intramuscular injections or oral administration of the drug.
Special groups of patients (general recommendations for both forms)
Elderly, debilitated patients: the use of the drug Suprastin ® requires special caution, because in these patients, antihistamines are more likely to cause side effects (dizziness, drowsiness).
Patients with impaired liver function: a dose reduction may be required due to a decrease in the metabolism of the active component of the drug in liver diseases.
Patients with impaired renal function: a change in the dosage regimen and a dose reduction may be required due to the fact that the active component is mainly excreted through the kidneys.
Overdose
Symptoms: hallucinations, anxiety, ataxia, incoordination, athetosis, convulsions. In young children - agitation, anxiety, dry mouth, fixed dilated pupils, facial flushing, sinus tachycardia, urinary retention, fever, coma.
In adults, fever and facial redness are observed intermittently, after a period of excitement followed by convulsions and post-convulsant depression, coma.
Treatment: up to 12 hours after taking the drug orally, gastric lavage is necessary (it should be taken into account that gastric emptying is prevented by the anticholinergic effect of the drug). The use of activated carbon is also indicated.
Monitoring of blood pressure and breathing parameters is necessary. Symptomatic therapy, resuscitation measures. A specific antidote is unknown.

Suprastin tablets contain 25 mg of chloropyramine hydrochloride, gelatin, lactose monohydrate, stearic acid, sodium starch glycolate type A, potato starch, talc.
Suprastin in ampoules contains chloropyramine hydrochloride and water as an auxiliary component.
Release form
The medicine has 2 forms: tablets 25 mg and solution d/i 20 mg/ml.
Suprastin tablets are disc-shaped, grayish-white or white. The name “SUPRASTIN” is engraved on one side and a line on the other. The smell is weak or completely absent. The tablets are packaged in blisters of 10 pieces, 2 blisters in a package.
The aqueous solution is transparent and colorless, with a slight specific odor. Available in ampoules with a capacity of 1 ml, package No. 5.

Pharmacodynamics and pharmacokinetics
The active substance of Suprastin is a chlorinated analogue of tripelenamine and belongs to the group of 1st generation antihistamines, which - due to the main side effect - are also called sedatives.
The mechanism of action of chloropyramine is due to the ability of this substance to block histamine type H1 receptors. Studies (both preclinical and clinical) prove its effectiveness against hay fever and other allergic diseases.
Like other H1 blockers, the drug's action is aimed primarily at preventing immediate hypersensitivity reactions, and in cases where the reaction has already begun to develop, at preventing further histamine release.
Experiments on guinea pigs showed that even in a very low dosage, the drug protected animals from the effects of histamine at a dose 120 times higher than the lethal dose.
Chlorpyramin also affects the central nervous system, capillary permeability and smooth muscle.
Absorption from the digestive canal after taking the required dose is rapid. The effect is observed within minutes after taking the tablet, reaches its maximum within 1 hour and lasts for up to 6 hours.
Chlorpyramin is metabolized in the liver. Metabolic products are excreted mainly by the kidneys. Moreover, the drug is eliminated from the body of a child faster than from the body of an adult.
In patients with renal dysfunction, the elimination of chloropyramine may be reduced. With liver dysfunction, the metabolism of the substance slows down, which requires changing the dose of the drug.

From the cardiovascular system, arrhythmia, tachycardia, and arterial hypotension are possible. From the digestive system: discomfort and pain in the epigastrium, dry mouth, diarrhea, nausea, vomiting, increased symptoms of gastroesophageal reflux, increased or complete lack of appetite.
On the part of the blood system, in rare cases, pathological changes in the blood may be observed (for example, agranulocytosis, hemolytic anemia, leukopenia, thrombocytopenia, etc.).

The duration of treatment depends on the severity of allergy symptoms and the body's sensitivity to chloropyramine. As a rule, the medicine is taken in short courses (5-7 days) when the process worsens. In case of severe disease, the doctor may make other prescriptions.
It should be remembered that with prolonged use of the drug, the patient may develop addiction to it.

The use of Suprastin is also advisable for chickenpox: the drug can reduce the intensity of itching and somewhat calm the child’s nervous system.
Before giving Suprastin to children, you should read the list of contraindications. The use of tablets is contraindicated in children under 3 years of age; the solution is not prescribed to newborn infants (including premature infants).
In addition, you should not give the medicine in any form during an acute attack of asthma, intolerance to chloropyramine or any of the auxiliary components.
The manufacturer does not produce special children's Suprastin with a lower dosage of the active substance. There are no other dosage forms (ointment, drops, syrup) except tablets and solution.
How should children take Suprastin tablets?
Dosage of Suprastin for children under 6 years of age: 1 tablet/day, divided into 2 doses. According to the instructions, for children 6-14 years old the dose is 1-1.5 tablets/day. (0.5 tablets at each dose).

Accidental or intentional overdose, especially in children, can cause death.
The symptoms and signs that occur with an overdose of Suprastin are similar to the symptoms of atropine poisoning. They are expressed in the form:

In some cases, sinus tachycardia, dry mouth, facial flushing, fixed dilated pupils, fever, and urinary retention occur.
In adults, facial flushing and fever are observed intermittently, with periods of agitation followed by convulsive seizures and postconvulsive depression. The patient develops coma, cardiac and respiratory failure. Death occurs within 2-18 hours.
Interaction
Simultaneous use with MAO inhibitors helps to prolong and enhance the anticholinergic effects of Suprastin.
With caution, the medicine is prescribed with: tranquilizers, sedatives (including sedative analgesics), muscarinergic parasympatholytics, atropine, tricyclic antidepressants. This is due to the fact that in combination with Suprastin these drugs exhibit synergistic action.
Alcohol potentiates the inhibitory effect of chloropyramine on the central nervous system, and therefore, during treatment with Suprastin, drinking alcohol should be avoided.
