Claritin or Zyrtec?

  • 1st generation - drugs Tavegil, Suprastin, Fenistil and Diphenhydramine;
  • 2nd generation – the drug Zyrtec;
  • 3rd generation – drugs Claritin, Telfast, Erius.

The first generation of drugs has an excellent antiallergic effect, which is combined with a strong side effect in the form of drowsiness. The second generation of antihistamines (Zyrtec) has a significantly smaller drowsiness effect, but also has a weaker antiallergic effect compared to the first generation drugs. But the third generation (Claritin, Telfast, Erius) has a pronounced antiallergic effect, like the first generation, combined with an almost complete absence of side effects in the form of drowsiness. Based on the above, the effectiveness of Claritin is higher and the side effects are less than that of Zyrtec.

Claritin: instructions for use for children and adults, analogues

Allergies are among the three most common diseases in the world. Manifesting itself as a rash, nasal congestion, or rarely swelling, it does not cause concern for everyone, but in vain. In extreme cases, in the absence of qualified medical care, an allergic reaction can cause death. One of the most effective medications used to combat its manifestations is Claritin. Effective and safe, it is suitable for the treatment and prevention of adults and children.

General information

Claritin is manufactured by the SCHERING-PLOUGH LABO NV corporation. The drug has a pronounced:

  • antiallergic;
  • antihistamine;
  • antipruritic actions.

The mechanism of its effect on the body is simple: it blocks histamine receptors within half an hour after ingestion, maintaining a therapeutic effect for up to 24 hours.

Note! Allergic manifestations are the result of the action of histamine on the body. This is a special substance that is released from cells when exposed to an allergen. Subsequently, histamine binds to histamine receptors, after which a series of reactions are triggered that directly provoke allergic manifestations. In other words, blood flow increases, the skin and mucous membranes become inflamed, the person begins to sneeze, cough, and sometimes choke.

Claritin acts exclusively on histamine receptors, preventing the initiation of reactions that lead to the development of allergic manifestations. Due to the fact that after it is ingested, some reactions are already advanced, the patient may feel itching or constantly sneeze for some time. Meanwhile, thanks to him, these symptoms quickly pass and relief comes.

The main advantages of Claritin are its versatility, since it is prescribed even for diseases accompanied by allergic reactions - bronchitis, bronchial asthma, and the absence of sedative effects. Unlike medications of the previous generation, for example, Suprastin, it does not affect the histamine receptors of the central nervous system, and therefore does not cause drowsiness and inhibition of reactions.

Claritin also eliminates skin hyperemia and strengthens the strength of the walls of blood vessels.

Release forms

You can buy Claritin in tablets or syrup. In both forms, the active ingredient is loratadine. One tablet contains 10 mg of loratadine, and 1 ml of syrup contains 1 mg.

You can recognize the tablets by their unusual oval shape and a specific sign in the form of a flask or bowl on one of the sides. The manufacturer recommends focusing on this symbol to eliminate the risk of receiving a counterfeit. For ease of separation, the tablets have a line in the middle.

The syrup is a clear, sometimes yellowish liquid and is most often prescribed for the treatment of children.

Important! In addition to the active substance, the tablets and syrup contain only components that are not capable of influencing the components of allergic reactions.

Indications

Claritin is used for treatment and prevention:

  • seasonal allergies, which manifest themselves as rhinitis, conjunctivitis, hay fever - a reaction to plant pollen;
  • allergic rhinitis that persists throughout the year;
  • chronic form of urticaria;
  • skin lesions of an allergic nature - dermatitis, eczema;
  • edema of various etymologies, including Quincke's edema;
  • allergies to insect bites;
  • pseudo-allergies provoked by the ingestion of histamine liberators - strawberries, chocolate, cocoa, tomatoes.

An allergic reaction is characterized by several symptoms that the medication effectively eliminates. This is about:

  • sneezing;
  • nasal congestion;
  • swelling of the mucous membranes and skin;
  • itching;
  • burning in the eyes;
  • rash;
  • bronchospasm.

Method of administration and dose

There are no recommendations regarding the timing of use of the drug. Tablets or syrup are drunk when signs of allergy appear, regardless of food intake. It is advisable to wash them down with clean water.

Important! The dosage should be prescribed by a qualified specialist, taking into account the age and presence of chronic diseases in the person.

Typically, children under 3 years of age are prescribed syrup, and older children are prescribed tablets. Moreover, giving Claritin before the child reaches 2 years of age is not recommended. Take the drug once a day at the same time. Children's dosage is determined by body weight and is calculated using one of two formulas:

  • If the baby's body weight has not reached 30 kg, 5 mg of Claritin is prescribed - this is 1 scoop or teaspoon or half a tablet.
  • If body weight is more than 30 kg, 10 mg of Claritin is prescribed, respectively, 2 spoons or a whole tablet.

Note! Sometimes a pediatrician may recommend taking 10 mg of Claritin at a time for a child who has not gained 30 kg, once every 2 days. In extreme cases, infants are prescribed 1.5 ml of syrup up to 12 months of age and 3 ml of syrup up to 2 years of age to eliminate signs of allergies.

Children over 12 years of age and adults are prescribed the drug in the form of tablets, which are taken one per day at the same time. The only exceptions are people suffering from liver disease or kidney failure. They are recommended to take 1 tablet once every 2 days or half a tablet every day.

The course of treatment is determined by the severity of symptoms and usually ranges from 7 to 14 days. When taking Claritin as a prophylactic agent, it can be extended for a month or more, but only after consultation with a specialist.

Important! It is recommended to stop taking the drug at least 2 days before allergy testing to avoid false results.

Use during pregnancy

There is no exact data regarding the effect of the active substance of the drug on the fetus. Experiments were carried out only with the participation of animals and showed the absence of negative effects. Based on them, a doctor can prescribe Claritin to a pregnant woman, but only if the benefits of taking it outweigh the possible harm.

It is also worth noting that the drug also passes into breast milk, and its concentration there remains quite high, as in the blood. In this regard, it is not prescribed during lactation.

Side effects

Children and adults may experience the following after taking Claritin:

Most often, these signs are the result of an overdose - taking more than 40 mg of loratadine at a time. Children may additionally experience disturbances in muscle tone, twitching or distortion of the lips, slow movements of the fingers, and trembling in the body.

Medical care for overdose is limited to gastric lavage and taking enterosorbents.

Analogues of Claritin

You can replace Claritin with other medications based on loratadine. Among them:

Analogues of the drug based on other active ingredients:

Storage conditions and periods

It is recommended to store Claritin in a dark place away from children, observing a suitable temperature regime: 2 - 30 C degrees. The tablets retain their properties for 4 years from the date of manufacture, the syrup for 3 years. You can't take them afterwards.

special instructions

Despite the fact that the drug is well tolerated, when using it for the first time it is better to avoid excessive activity and not drive a vehicle. Selectivity of action, safety, effectiveness and relatively low price have made it one of the most popular medications in more than 100 countries around the world. Meanwhile, doctors do not advise abusing it. For the treatment of short-term allergic manifestations, a single dose is sufficient. At the same time, if it is necessary to transfer the disease into a state of remission, it may take from 3 months to a year, during which the course of treatment is not interrupted.

Claritin is a proven antiallergic drug recommended for adults and children. If the dosage and frequency of administration are observed, it quickly eliminates allergy symptoms and returns to normal life.

I was bitten by a wasp in the summer. I knew it would hurt, but I didn’t expect such a reaction. The shoulder swelled three times. At the emergency room they prescribed fenistil gel for the bite site and a course of Claritin. After 3 days, the shoulder was doubled in size. At home my grandmother already made some kind of emulsion based on herbs and I was completely cured))

This spring I have severe allergies, I bought Claritin, hoping that it would help. rnSo, there is no effect from it, a terrible condition and a terrible runny nose. rnWhen it comes to serious allergies, Claritin does not work at all!

A new generation medicine - Claritin syrup: instructions for use for children of all ages

Many parents know how difficult it is to choose an effective and safe medicine to treat allergies in their son or daughter. Claritin syrup for children is one of the few drugs successfully used in pediatric practice. The new generation drug not only actively eliminates negative symptoms, but also rarely causes side effects.

The components of the medicinal syrup, which has a pleasant peach taste, have a positive effect on the child’s body with acute and chronic forms of allergies. Before starting therapy, parents should learn the rules for using an antihistamine.

Composition of antihistamine syrup

Loratadine is the active ingredient in the antiallergic syrup Claritin. A transparent liquid of a slight yellowish tint with an antihistamine effect contains purified water, glycerol, sodium benzoate, propylene glycol. Peach flavoring gives the medicinal syrup a pleasant taste.

The product is bottled in dark glass bottles, the volume of the drug is 60 and 120 ml. For ease of use, each package contains a dispensing spoon made of non-toxic plastic.

Action

Therapeutic syrup has a complex effect on the body. The product is suitable for eliminating acute symptoms and preventing severe relapses in the chronic form of an allergic reaction.

After taking Claritin, not only the H1-blocking properties of the drug appear, but also a complex effect on the body, reducing susceptibility to the effects of allergens.

After taking the syrup, selective blocking of H1-peripheral histamine receptors leads to a weakening of allergy symptoms within 30 minutes. The medicine is completely absorbed within one and a half to two hours, the greatest antihistamine effect is achieved 8–12 hours after consuming the drug. Loratadine is metabolized into desloratadine, the active effect of the drug lasts 24 hours, sometimes longer.

Pediatricians from different countries consider Claritin one of the safest drugs for treating various forms of allergies in children. For more than 15 years, doctors have often prescribed a medicine based on loratadine to combat allergies in young patients.

Contraindications

Restrictions for taking syrup with loratadine:

  • excessive sensitivity to the ingredients of the drug;
  • child's age up to two years;

Claritin - instructions for use for children and adults, indications, composition, side effects, analogues and price

According to WHO statistics, from 30 to 60 percent of the world's population are susceptible to allergies. This is due to environmental pollution, the use of synthetic additives in food production, and the development of the household chemical industry. But at the same time, pharmacology is also developing, offering reliable and safe medications for allergies. An effective remedy for reducing histamine, treating seasonal runny nose, relieving swelling, rashes and itching is Claritin - the instructions for use will tell you in detail about the use and contraindications of the drug.

Claritin for allergies

The main task of anti-allergenic drugs and antihistamines is to block H1 receptors and reduce the level of histamine in the blood. But, in addition, it is also necessary to eliminate the accompanying skin and respiratory allergic symptoms of the disease:

  • cough;
  • itching;
  • runny nose;
  • swelling;
  • skin diseases - rash, redness;
  • tearfulness;
  • inflammation.

Most antiallergens only block histamine H1 receptors, and to eliminate other allergy symptoms it is necessary to take other medications that negatively affect the organs and general health of a person. Claritin solves all problems comprehensively, and can also be used for preventive purposes. According to the American pharmacological company IMS, it is the most prescribed medicine for allergies. The drug belongs to the latest generation of selective blockers; it is used and recommended by doctors in more than a hundred countries around the world.

Composition and release form

Claritin is sold in the form of tablets and syrup:

The syrup is colorless and transparent, sometimes with a golden tint. Without impurities and sediment

Distilled water, citric acid, sodium benzoate, glycerol, propylene glycol, sucrose, peach synthetic flavor

Pharmacodynamics and pharmacokinetics

The active substance loratadine is an antihistamine tricyclic chemical compound that has a blocking effect on peripheral H1 receptors. The antiallergic effect of Claritin begins within half an hour after ingestion, and the main symptoms of the disease disappear. The maximum antihistamine effect is achieved after 8-12 hours and lasts for 24 hours. Does not affect the nervous system, does not have a sedative effect. With long-term use, no significant deterioration in the vital functions of the body is observed.

Quickly absorbed into the gastrointestinal tract. If you take the medicine before meals, the maximum concentration of the substance will occur an hour later, but the overall level of loratadine concentration in the blood is not affected by food intake. The maximum concentration increases only in elderly people, with chronic or terminal renal failure, and liver diseases caused by alcohol. During metabolism, part of loratadine is produced into the selective blocker desloratadine. Both components are excreted in the form of conjugated final metabolites in urine and bile:

  • The half-life of loratadine ranges from 3 to 20 hours;
  • The half-life of desloratadine is from 8 to 92 hours.

Indications for use

It is used for the following allergic reactions and diseases:

  1. Chronic idiopathic urticaria.
  2. Various dermatological diseases and other allergic skin reactions.
  3. For seasonal or year-round allergic rhinitis to eliminate:
  • runny nose;
  • sneezing;
  • inflammation and itching of the nasal mucosa.
  1. For seasonal or year-round allergic rhinoconjunctivitis (hay fever) eliminates:
  • lacrimation;
  • itching and burning in the eye area.

Directions for use and dosage

Claritin is taken orally, before or after meals. Taking the medicine does not depend on the time of eating; the only thing that needs to be taken into account is that the effect of the medicine taken on an empty stomach will begin an hour earlier than after eating. Claritin in any form is contraindicated for children under 2 years of age. It is necessary to limit the dosage of the medication for people suffering from serious diseases and liver pathologies. In such cases, it is recommended to consult a doctor.

Pills

The dosage for adults and children over 12 years of age is 10 ml once a day - that’s 1 tablet. The same dose of medication can be taken by elderly people, patients with chronic and end-stage renal failure. For children from 3 to 12 years old, the dose is calculated depending on body weight. A child whose weight is 30 kilograms or more is given 1 tablet (10 ml) 1 time per day, with a lower weight and children from 2 to 3 years old, tablets are prohibited.

Syrup

For adult patients, syrup is prescribed two teaspoons (10 ml) once a day. For children from two to three years old and weighing less than 30 kilograms, the permissible dose of syrup is 1 teaspoon or 5 ml per day. For people, including children weighing more than 30 kilograms, with severe liver damage and impairment of its functions, the permissible initial dose is no more than 10 ml of the drug (2 teaspoons) once every two days. If for the same diseases the body weight is less than 30 kilograms, the initial dosage will be half as much - 5 ml (1 teaspoon of syrup) every 48 hours.

special instructions

You should not take Claritin less than 48 hours before taking allergy skin tests (allergy tests). Because antihistamines contained in the drug can distort the results of the analysis, which will not allow a correct diagnosis. The drug has a sedative effect and does not reduce concentration or psychomotor reactions. The drug does not have a direct contraindication to driving a car or other mechanical means, but sometimes the medication can cause drowsiness and have a sedative effect.

During pregnancy

Clinical studies have not clearly established the safety of taking Claritin during pregnancy. Pregnant women are allowed to take the medicine in special cases if the expected benefit for the woman is higher than the potential danger to the health of the fetus. Claritin is not recommended during breastfeeding, because the active substances of the drug (loratadine metabolite) partially enter the mammary glands and breast milk. In this case, a doctor's recommendation is required.

For children

According to experts, Claritin is the safest anti-allergen for children's bodies. It is used by pediatricians in many countries. When taking other similar medications for more than 10 days, the body begins to get used to them. The effectiveness of treatment decreases, and the risk of side effects increases, which are sometimes impossible to predict in a child’s body. The drug does not have a harmful effect on the child’s health. Children's Claritin is prescribed both for short-term treatment for acute attacks of allergies, and for long-term therapy for relapses of the disease - up to 12 months.

Drug interactions

During clinical studies of Claritin, no particularly significant interactions of its components with other medical or pharmacological drugs were found. There are some reactions of loratadine (a slight increase in its dose in the patient’s blood) while taking Claritin and antibiotics, an antifungal agent. A minor and harmless reaction occurs when interacting with the following medications:

Claritin and alcohol

The active and auxiliary components of Claritin, which are metabolized after taking the drug desloratadine, do not penetrate the human brain centers. The drug has no effect on the central nervous system and does not enhance the harmful effects of alcohol. But you need to take into account that this is general data, and everyone’s body characteristics are different. In each individual case, it is impossible to accurately predict what the consequences of the interaction of medication and alcohol may be. This applies to any medication.

Side effects

Like any medicine, Claritin has certain side effects, but they do not exceed the effects on the body of a placebo drug (a harmless substance manufactured under the guise of a medicine to create the effect of self-hypnosis that it can cure a disease). The frequency of side effects of Claritin does not exceed 2%; according to patient reviews, in rare cases the following may be observed:

  • dry mouth;
  • nausea;
  • skin rash;
  • short-term dizziness;
  • drowsiness was noted;
  • cardiopalmus;
  • nervousness.

Overdose

With a slight increase in dose, there is no negative reaction or effect on the body. Adverse effects for an adult can only be caused after increasing the dosage by at least four times (4 mg of loratadine), in children after taking a dose of the drug twice the recommended dose. In case of an overdose of the drug, adsorbents, gastric lavage with a 10% solution of sodium chloride (table salt), and activated carbon are prescribed. Overdose symptoms:

  • painful palpitations;
  • headache;
  • loss of strength, drowsiness.

Contraindications

Claritin is contraindicated in case of high individual sensitivity of the patient to the components of the drug, especially to loratadine. Reasons for refusing to take medication may include:

  • Hereditary diseases associated with low levels of Lapp lactase in the body.
  • If the body reacts negatively to galactose, which is part of Claritin tablets. In this case, syrup will do.
  • During breastfeeding.
  • Age up to two years for syrup, and up to three years for tablets.

Terms of sale and storage

Available in pharmacies without a prescription. Store out of the reach of children at a temperature of 2 to 25 degrees Celsius. Keep the syrup in a dark place, away from direct sunlight. Shelf life for tablets is 4 years, syrup – 3 years.

Analogs

Claritin has no absolute analogues regarding the complex treatment of allergies, the period of taking the medicine, the risks of formation and development of side effects. But in terms of the presence of loratadine in the composition and the angistaminic effect it corresponds to:

Lidia Ivanovna, 52 years old

I was allergic to medications for stomach ulcers. A rash appeared on my knee and became so swollen that it was painful to walk. The result of Claritin was after the first pill: the itching and inflammation went away, I was able to move around without pain. On the third day, all symptoms disappeared altogether. The great thing is that it does not cause drowsiness like suprastin, which I once used to treat tracheitis.

It is not clear why, about five years ago an allergy to cat hair appeared. If I go to a friend who has a cat, then skin manifestations (rashes and blisters) are guaranteed. I tried different tablets and drops, but only Claritin really helps. I take a pill an hour before coming to the apartment where the cat is, and everything is fine - no hives.

The first time I chose Claritin by chance, the pharmacy gave me a good review. I had a severe allergy attack - my eyes itched, I began to choke, as if I had asthma. Helped quickly. Literally after 15 minutes it becomes much easier. But it does make you drowsy. And it doesn’t last for 24 hours, as the annotation says, but for 12 hours. I don’t know, maybe it depends on the body.

Claritin syrup for children

Claritin is known as an effective allergy remedy. Adults take it both during attacks and to prevent them, and especially for children, the drug is produced in the form of syrup. At what age can it be used, when does it help and in what dosage is it used in children?

Release form

Claritin syrup is a sweet, thick liquid, which comes in 60 or 120 milliliters in one glass bottle. It is transparent, without additional inclusions, yellowish or colorless and smells like peach. The packaging includes a plastic spoon or graduated syringe.

In addition to syrup, the drug is also available in tablets.

Compound

The main component of Claritin is called loratadine. This substance is contained in 1 ml of syrup at a dose of 1 mg. During production, glycerol, sodium benzoate, purified water, citric acid and propylene glycol are added to it. The sweetness of the medicine is given by sucrose, and the smell is given by peach flavoring.

Operating principle

Loratadine acts on peripheral histamine receptors and blocks them, which prevents them from binding to the main mediator of the allergic reaction - histamine. That is why this action is called antihistamine. With Claritin syrup it is very long-lasting and fast - it begins to appear within half an hour after taking it and lasts more than a day.

The use of the medicine reduces allergic inflammation and effectively eliminates itching, lacrimation, runny nose and other signs of allergy.

Indications

Claritin is prescribed to a child:

  • For allergic rhinitis, which is manifested by itching of the nasopharyngeal mucosa, sneezing and profuse nasal discharge.
  • For allergic conjunctivitis, the symptoms of which are usually itching or burning in the eyes and severe watery eyes.
  • For urticaria.
  • For eczema and other allergic skin diseases.
  • For chickenpox, to relieve severe itching of the blisters.
  • For Quincke's edema.
  • If you have an allergic reaction to insect bites.

At what age is it allowed to take it?

Claritin in syrup form is prescribed from the age of two. The tablet medicine is prescribed to children over 3 years of age. In rare cases, the medication can be used in children under 2 years of age, but only for serious reasons and under the supervision of a pediatrician, who selects the dose of the drug individually.

Contraindications

The syrup should not be taken in the following situations:

  • If your child is intolerant to loratadine or another ingredient in the medicine.
  • In case of malabsorption of glucose and galactose or lack of isomaltase and sucrase, since the composition of the medication includes sucrose.

For children with liver pathologies, the medicine is prescribed with caution, changing the dosage regimen.

Side effects

Since loratadine has no effect on the central nervous system, taking the syrup does not cause drowsiness. In addition, treatment with Claritin does not worsen heart function. However, the medication sometimes causes negative symptoms such as headaches, increased fatigue and nervousness. In rare cases, the drug impairs sleep, causes allergies, or affects the functioning of the digestive system.

Instructions for use and dosage

  • The drug is given once a day at any time, regardless of meals, dosing it with the plastic teaspoon present in the package (1 spoon holds 5 ml of syrup) or with a graduated plastic syringe (it is also designed for 5 ml of the drug). The product can be washed down with clean water.
  • For a child aged 2 to 12 years, the dose of syrup is determined by body weight. If the weight of a small patient is less than 30 kg, then the medication is given in 5 ml doses. If you weigh more than 30 kg, a single dosage will be 10 ml of syrup.
  • A child over 12 years old is given Claritin 10 ml per dose.
  • For liver diseases, the dosage is not reduced, but the drug is given not daily, but every other day.
  • The duration of administration is influenced by the speed of elimination of allergy symptoms. The drug can be prescribed for several days or in a course lasting 1-2 weeks.

Overdose

If a child accidentally takes more medicine than the doctor prescribed, it will cause increased heart rate, drowsiness, and headaches. In such a situation, it is necessary to immediately show the patient to a doctor, rinse the stomach, give a drug from the group of sorbents and prescribe other necessary treatment.

Terms of sale and storage

You do not need a doctor's prescription to purchase Claritin at a pharmacy. The average price of a bottle with 60 ml of medicine is 250 rubles. Storing the syrup at home is recommended at room temperature in a place where the product will be inaccessible to a small child. The shelf life of this form of medicine is 3 years. If it has expired, it is unacceptable to give the product to children.

Reviews

Most parents are satisfied with the use of Claritin in children with allergies and note that this remedy quickly eliminates both the consequences of an immediate allergic reaction (for example, to an insect bite) and the manifestations of chronic pathology. At the same time, it does not dry out the mucous membrane and does not provoke drowsiness, which distinguishes it favorably from antihistamines of the first generations.

The syrup is also praised for its pleasant taste and is considered the most convenient form for treating children 2-3 years old. In addition, it is often prescribed to small patients weighing up to 30 kg, even if they are already 3 years old, since the single dosage for them is 5 mg. Since this is a half tablet and if you break it, you may get the wrong dose, it is better to prefer syrup.

Negative reviews of the drug most often mention its high cost. In addition, sometimes taking Claritin turns out to be ineffective and the syrup does not eliminate unpleasant symptoms, which is why you have to buy another antihistamine.

Analogs

Claritin can be replaced by other drugs based on loratadine, for example, Lomilan, Clarisens, LoraHEXAL, Loratadine STADA, Claridol and others. Many of them come in a suspension or syrup, so they can be a complete replacement. In addition, the doctor may prescribe an antihistamine with a different composition for the child.

Antiallergic drug, selective blocker of peripheral histamine H1 receptors . Loratadine is a tricyclic compound with a pronounced antihistamine effect. Has a quick and long-lasting antiallergic effect.

Loratadine does not penetrate the BBB and has no effect on the central nervous system. Does not have a clinically significant anticholinergic or sedative effect, i.e. does not cause drowsiness and does not affect the speed of psychomotor reactions when used in recommended doses. Taking Claritin does not lead to prolongation of the QT interval on the ECG. No clinically significant changes in vital signs, physical examination, laboratory results, or ECG were observed during long-term treatment.

Loratadine does not have significant selectivity for histamine H2 receptors . Does not inhibit norepinephrine reuptake and has virtually no effect on the cardiovascular system or pacemaker function.

After oral administration of the drug Claritin, the onset of action is within 30 minutes. The antihistamine effect reaches its maximum 8-12 hours after the onset of action and lasts more than 24 hours.

After taking the drug orally, loratadine is quickly and well absorbed from the gastrointestinal tract. Tmax of loratadine in blood plasma is 1-1.5 hours, and its active metabolite desloratadine is 1.5-3.7 hours. Eating increases Tmax of loratadine and desloratadine by approximately 1 hour, but does not affect the effectiveness of the drug. Cmax of loratadine and desloratadine does not depend on food intake.

The bioavailability of loratadine and its active metabolite is dose-dependent.

Loratadine is highly bound to plasma proteins 97-99%, and its active metabolite is bound to a moderate extent.

Loratadine is metabolized to desloratadine with the participation of the CYP3A4 isoenzyme and, to a lesser extent, CYP2D6.

Excreted by the kidneys (approximately 40% of the oral dose) and intestines (approximately 42% of the oral dose) for more than 10 days, mainly in the form of conjugated metabolites. Approximately 27% of an oral dose is excreted by the kidneys within 24 hours after taking the drug. Less than 1% of the active substance is excreted unchanged by the kidneys within 24 hours after taking the drug.

T1 /2 of loratadine ranges from 3 to 20 hours (average 8.4 hours), and desloratadine - from 8.8 to 92 hours (average 28 hours).

Pharmacokinetics in special clinical situations

The pharmacokinetic profiles of loratadine and its active metabolite were comparable in adults and elderly healthy volunteers.

T1 /2 of loratadine and desloratadine in elderly patients ranges from 6.7 to 37 hours (average 18.2 hours) and from 11 to 39 hours (average 17.5 hours), respectively.

In patients with chronic kidney disease, the Cmax and AUC of loratadine and its active metabolite are increased compared to patients with normal renal function. T1 /2 of loratadine and its active metabolite does not differ from those in healthy patients. T1 /2 of loratadine and its active metabolite does not change in the presence of chronic renal failure. Hemodialysis in patients with chronic renal failure does not affect the pharmacokinetics of loratadine and its active metabolite.

In patients with alcoholic liver damage, the Cmax and AUC of loratadine and its active metabolite increase by 2 times compared to these indicators in patients with normal liver function. T1 /2 of loratadine and its active metabolite increases with alcoholic liver damage (depending on the severity of the disease) and does not change in the presence of chronic renal failure.

- seasonal (hay fever) and year-round allergic rhinitis and allergic conjunctivitis (to eliminate the symptoms associated with these diseases - sneezing, itching of the nasal mucosa, rhinorrhea, burning and itching in the eyes, lacrimation);

- chronic idiopathic urticaria;

- skin diseases of allergic origin.

- age up to 2 years (for syrup);

- age up to 3 years (for tablets);

- lactation period (breastfeeding);

- rare hereditary diseases (impaired galactose tolerance, lapp lactase deficiency or glucose-galactose malabsorption) - due to the presence of lactose included in the tablets;

- sucrase/isomaltase deficiency, fructose intolerance, glucose-galactose malabsorption - due to the presence of sucrose included in the syrup;

- hypersensitivity to the components of the drug.

with caution to patients with severe liver dysfunction and during pregnancy.

The drug is prescribed orally, regardless of food intake.

Adults (including elderly patients) and adolescents over the age of 12 years are recommended to take Claritin at a dose of 10 mg (1 tablet or 2 teaspoons /10 ml/ syrup) 1 time / day.

For children aged 2 to 12 years, the dose of Claritin is recommended to be prescribed depending on body weight: for body weight less than 30 kg - 5 mg (1/2 tablet or 1 teaspoon /5 ml/ syrup) 1 time / day, for weight body 30 kg or more - 10 mg (1 tablet or 2 teaspoons /10 ml/ syrup) 1 time / day.

For adults and children weighing more than 30 kg with severe liver dysfunction, the initial dose is 10 mg (1 tablet or 2 teaspoons /10 ml/ syrup) every other day, with a body weight of 30 kg or less - 5 mg (1 teaspoon spoon /5 ml/ syrup) every other day.

In elderly patients and in patients with chronic renal failure, no dose adjustment is required.

In clinical studies

In clinical studies involving children aged 2 to 12 years taking Claritin, headache (2.7%), nervousness (2.3%), and fatigue (1%) were observed more often than in the placebo group.

From the nervous system: in children aged 2 to 12 years - headache (2.7%), nervousness (2.3%), fatigue (1%); in adults - headache (0.6%), drowsiness (1.2%), insomnia (0.1%).

From the digestive system: in adults - increased appetite (0.5%).

In the post-marketing period

From the nervous system: very rarely (< 1/10,000) - dizziness, fatigue.

From the digestive system: very rarely (< 1/10,000) - dry mouth, gastrointestinal disorders (nausea, gastritis), impaired liver function.

Allergic reactions: very rarely (< 1/10,000) - rash, anaphylaxis.

From the cardiovascular system: very rarely (< 1/10,000) - palpitations, tachycardia.

From the skin: very rarely (< 1/10,000) - alopecia.

Symptoms: drowsiness, tachycardia, headache.

Treatment: gastric lavage, taking adsorbents (crushed activated carbon with water), symptomatic and supportive therapy. Loratadine is not eliminated by hemodialysis. After emergency care is provided, it is necessary to continue monitoring the patient's condition.

Claritin does not enhance the effect of ethanol (alcohol) on the central nervous system.

When taking Claritin together with ketoconazole, erythromycin or cimetidine, an increase in the plasma concentration of loratadine was observed, but this increase was not clinically significant, incl. according to ECG data.

Claritin should be discontinued 48 hours before skin testing, as antihistamines may interfere with diagnostic test results.

Use in pediatrics

Children aged 2 to 3 years are recommended to take the drug Claritin in the form of syrup.

Impact on the ability to drive vehicles and operate machinery

No negative effects of the drug Claritin on the ability to drive a car or perform other activities requiring increased concentration have been identified. However, in very rare cases, some patients experience drowsiness while taking Claritin, which may affect their ability to drive or operate machines.

The safety of loratadine during pregnancy has not been established. The use of Claritin during pregnancy is possible only if the expected benefit to the mother outweighs the potential risk to the fetus.

Loratadine and its active metabolite are excreted in breast milk, therefore, when prescribing the drug during lactation, the issue of stopping breastfeeding should be addressed.

Contraindications: age under 2 years (for syrup); age up to 3 years (for tablets).

Children aged 2 to 3 years are recommended to take the drug Claritin in the form of syrup.

The drug should be stored out of the reach of children at a temperature not exceeding 25°C. The shelf life of tablets is 4 years, syrup - 3 years.
