Fluimucil-antibiotic IT is a combined drug with antibacterial and mucolytic action.
Release form and composition
- lyophilisate for the preparation of a solution for injection and inhalation: a plate-like mass of pale yellow or white color with a faint sulfuric odor (in a colorless glass bottle complete with solvent in an ampoule of 4 ml, 3 sets in a plastic tray, 1 tray in a cardboard box);

Thiamphenicol glycinate acetylcysteinate is a complex compound that includes the antibiotic thiamphenicol and the mucolytic acetylcysteine. After absorption, this compound is split into its constituent substances - thiamphenicol and acetylcysteine.
Thiamphenicol is part of the group of chloramphenicol derivatives; its mechanism of action is associated with inhibition of bacterial cell protein production. The substance is a broad-spectrum antibacterial drug. Thiamphenicol in vitro demonstrates effectiveness against bacteria that most often cause respiratory tract infections, both gram-positive (Corynebacterium diphtheriae, Streptococcus pneumoniae, Streptococcus pyogenes, Staphylococcus spp., Clostridium spp., Listeria spp.) and gram-negative (Neisseria spp., Haemophilus influenzae, Escherichia coli, Salmonella spp., Bordetella pertussis, Shigella spp., Brucella spp., Yersinia pestis, Bacteroides spp.).
As a result of the rupture of disulfide bonds of mucoproteins, acetylcysteine effectively and quickly dilutes sputum and pus, reduces their viscosity and facilitates discharge. Acetylcysteine promotes the penetration of thiamphenicol into lung tissue and prevents the process of bacterial adhesion to the surface of the epithelium of the respiratory tract mucosa.
Pharmacokinetics
Thiamphenicol
Thiamphenicol is characterized by rapid distribution in the body and accumulation in therapeutic concentrations in the tissues of the respiratory tract. After intramuscular (IM) administration, the maximum concentration (Cmax ) is observed after 1 hour.
The binding of the substance to plasma proteins does not reach 20%. The half-life (T ½ ) is approximately 3 hours, the volume of distribution is 40–68 l. Excreted by the kidneys by glomerular filtration; one day after administration, the amount of unchanged substance in the urine can vary from 50 to 70% of the initially administered dose. Penetrates through the placental barrier.
Acetylcysteine
The substance is quickly distributed in the body, penetrating into the intercellular space, mainly accumulating in bronchial secretions, lungs, kidneys and liver. T ½ is 2 hours. During the deacetylation process in the liver, the drug is transformed into cysteine. It is characterized by a fluid balance in the blood of free and plasma protein-bound acetylcysteine, as well as the products of its metabolic transformation (diacetylcysteine, cystine and cysteine). In the form of inactive metabolites (diacetylcysteine, inorganic sulfates), the drug is excreted by the kidneys, and in small quantities unchanged through the intestines. The substance penetrates the placenta.
Indications for use
- lesions of the lower respiratory tract: bronchitis in acute and chronic forms, prolonged pneumonia, emphysema, lung abscess, bronchiolitis, cystic fibrosis, bronchiectasis, whooping cough;
- diseases of the ENT organs and upper respiratory tract: laryngotracheitis, sinusitis, exudative otitis media;
- bronchopulmonary complications arising after thoracic surgical interventions (treatment and prevention): atelectasis, bronchopneumonia;
- obstructive and infectious complications of tracheostomy (treatment and prevention);
- concomitant nonspecific forms of respiratory infections, including cavernous lesions, mycobacterial infections (in order to improve drainage).
Fluimucil-antibiotic IT is also recommended for use in preparation for bronchoaspiration and bronchoscopy.
Contraindications
- thrombocytopenia;
- leukopenia;
- anemia;
- hypersensitivity to any of the components of the drug.

- chronic renal failure;

Instructions for use of Fluimucil-antibiotic IT: method and dosage
Fluimucil antibiotic IT is administered intramuscularly and used for inhalation, cavity rinsing, and applications.
Recommended dosage regimen depending on the route of administration of the drug:
- endotracheally (through a tracheostomy, endotracheal tube, bronchoscope): 1–2 ml of solution once a day; for children, 250 mg of lyophilisate is diluted in 4 ml of the supplied solvent, for adults – 500 mg;
- intranasally, instillation into the ears (for lesions of the nasopharynx and ear): inject 2-4 drops into each external auditory canal or nasal passage;
- inhalation: 1–2 times a day for children 125 mg, adults – 250 mg;
- washing of sinuses and cavities after surgical interventions in the area of the mastoid process and nose: 1–2 ml of solution per 1 procedure; for children, 250 mg of lyophilisate is diluted in 4 ml of the supplied solvent, for adults – 500 mg;
- IM: children under 3 years old – 125 mg 2 times a day; children from 3 to 7 years old – 2 times a day, 250 mg; children from 7 to 12 years old – 3 times a day, 250 mg; adolescents from 13 to 15 years old – 2 times a day, 500 mg; for adolescents over 15 years of age and adults – 2-3 times a day, 500 mg.
In newborns up to 14 days of life and premature infants, the average daily dose is 25 mg per 1 kg of body weight.
If necessary, in patients with severe disease in the first 2-3 days of the course, the dose may be increased by 2 times, with the exception of patients over 65 years of age, newborns and premature infants. The duration of therapy is no more than 10 days.
When using Fluimucil-antibiotic IT for inhalation, immediately before the procedure, it is necessary to remove the protective aluminum cap from the bottle with the lyophilisate and, having opened the ampoule with the solvent, inject the latter using a syringe into the closed bottle with the lyophilisate. After thoroughly mixing the contents of the bottle, take the recommended dose of the resulting solution with a syringe and pour it into the nebulizer (inhaler) reservoir. Inhalation is carried out for 5–10 minutes using a mask or nasal attachments.
The resulting solution of Fluimucil-antibiotic IT for inhalation can be stored for 24 hours at a temperature of 5 °C.
Side effects
During treatment with the drug, allergic reactions may develop.
When prescribing intramuscular administration of thiamphenicol glycinate acetylcysteinate, a slight burning sensation at the injection site may be observed, and rarely, leukopenia, anemia, reticulocytopenia, thrombocytopenia, neutropenia.
Inhalation administration of Fluimucil antibiotic IT can cause disorders such as local irritation of the respiratory tract, reflex cough, rhinitis, stomatitis, nausea. In some cases, bronchospasm develops, which requires the use of bronchodilators.
Overdose

How to dilute for inhalation?
For adults, 500 mg of dry substance is dissolved in 4 ml of water for injection and divided into 2 doses per day (2 ml each). Inhalations for children are prepared in the same amount of water for injection as for adults (4 ml), dissolving 250 mg of powder and also used twice a day.
Locally for washing the paranasal sinuses, incl. after surgical interventions, for endotracheal administration through a tracheostomy, bronchoscope or endotracheal tube, the powder is dissolved using water for injection in an amount of 4 ml, 500 mg for adults and 250 mg for children.
For otitis and diseases of the nasopharynx, 2-4 drops are instilled into the nasal passage or ear canal.
In case of appropriate indications, the dose of the drug can be doubled.
In newborns, premature and elderly patients, do not increase the dose.
The instructions for the antibiotic determine the course of treatment with the drug for no more than 10 days.
Overdose
Possible increased side effects of the drug, superinfection, disturbances of the bacterial flora. Symptomatic therapy is used as treatment.
Interaction

Thiamphenicol is a derivative of chloramphenicol, the mechanism of action is associated with inhibition of bacterial cell protein synthesis. Interaction with other drugs

During treatment, peripheral blood patterns should be monitored. If the number of leukocytes (less than 4 thousand/μl) and granulocytes (by more than 40%) decreases, the drug is discontinued.
