Mechanism of action of Derinat

Instructions for the drug Derinat: purpose, dosage and side effects

The patient's health largely depends on the state of his immunity. Strong immunity can resist various damaging factors, both infectious and tumor in nature.

Table of contents:

Some substances have the ability to “stimulate” the patient’s own immunity. In order to quickly and more successfully cope with a number of severe or protracted chronic diseases, modern medicine uses special drugs - immunomodulators. One of these popular remedies is Derinat.

Composition and properties of Derinat

Derint is available in different forms and has a wide spectrum of action.

This drug is an immunomodulatory agent that can affect cellular and humoral immunity. The main active ingredient is sodium deoxyribonucleate, which helps the body fight various types of infections and enhances its regenerative functions.

It helps restore the drainage function of the lymphatic system and protects cells from the harmful effects of radiation and chemotherapy drugs. Derinat contains sodium chloride and sterile distilled water as excipients.

This drug is devoid of carcinogenic effects and is not capable of destroying cells, therefore Derinat, whose side effects are minimal, is actively used to treat adults and children. The drug is usually well tolerated and has a pronounced positive effect on the patient's condition.

Since Derinat is used to stimulate the patient’s own immunity, the scope of its application is very wide.

It is used as part of complex therapy for viral, bacterial or fungal infections, in the treatment of various diseases accompanied by serious intoxication, complicated by poor function of the lymphatic system. This remedy stimulates the functioning of the lymph nodes and the entire system as a whole, helping to improve drainage and remove toxins. Derinat also helps to reduce the susceptibility of cells and tissues of the body during the treatment of cancer. It contributes to the patient's resistance to the negative effects of therapeutic radiation and chemotherapy. With this drug, the patient tolerates such difficult and intensive treatment much easier.

Release form, storage and shelf life

This drug is available in the form of drops and solutions for parenteral administration, as well as a product for topical use. Available in ampoules and glass bottles of 2, 5, 10 and 20 ml. You can also find Derinat on sale in bottles equipped with a dropper or a special spray for topical use, which greatly simplifies the use of the drug.

The shelf life of the medicine is within 5 years. Store Derinat at 4 – 18°C, protecting from the influence of light, high humidity and temperature changes.

It should be noted that Derinat is an additional drug in the complex treatment of diseases, especially severe and chronic ones. It combines well with other medications, but only a doctor can decide on the rationality of using the medicine, dosage and duration of treatment.

Application Features

The injection drug is administered intramuscularly, but subcutaneous injections are sometimes practiced. Drops are instilled into each nasal passage, and the liquid for external use is used in the form of irrigation of damaged areas of the body by spraying or application of the drug is performed.

Each type of disease requires its own duration of treatment and regularity of injections:

  • For malignant tumors and metastases, the doctor usually prescribes a course of 3 to 10 injections in a single dose, depending on the type and severity of the disease. Injections are given every 2 or 3 days.

Derinat in drops - dosage for children and adults

Side effects

Derinat solution for local and external use

With normal dosage, the medicine is usually very well accepted by patients, both adults and children. Therefore, Derinat, whose side effects are minimal, is widely used in medical practice, including for the treatment of children.

Possible side effects of the drug include a short-term rise in temperature during injections. Typically, this phenomenon develops within about three hours after administration of the medicine. Also, sometimes minor pain is observed at the injection site, which goes away on its own.

An increase in temperature is also a temporary phenomenon that does not require specific treatment or discontinuation of Derinat. If the patient is feeling unwell, non-steroidal anti-inflammatory and painkillers, for example Diphenhydramine, can be prescribed.

It should be used with caution in patients with diabetes. When using Derinat, they may experience hypoglycemia, so constant monitoring of the condition will be required.

Contraindications

The main contraindications to the use of this drug include the following:

  • Individual intolerance to Derinat, allergic reactions when using it.
  • Use during pregnancy and lactation

Despite the fact that Derinat has minor side effects, its use during pregnancy is considered undesirable, since the use of any medications during this period poses a certain threat to the pregnant woman and the fetus. During pregnancy and breastfeeding, Derinat can be prescribed by a doctor only if he considers that the possible harm from the drug will be much less than the benefit it brings.

Derinat easily penetrates all barriers of the body and can influence the formation and development of the unborn child.

Using this drug on your own in any condition, not just during pregnancy or breastfeeding, can be very risky. Only a specialist with a medical education can decide on the need to take this drug and prescribe it to the patient. Strict adherence to the instructions guarantees the correct effect and a positive effect on the patient’s health.

The drop form of the drug can be used without restrictions during pregnancy, and the injection form can be used in consultation with the doctor. unless it is really necessary for some reason. The main area of application for pregnant women is the prevention of acute respiratory viral infections and the treatment of mild and moderately severe forms.

Derinat injections: instructions for use

Solution for parenteral administration Derinat is a drug that belongs to the pharmacological group of immunomodulators. This drug is used in complex therapy of various pathologies accompanied by impaired functional activity of the immune system.

Release form and composition

Solution for parenteral administration Derinat is a colorless transparent liquid. The main active ingredient of the drug is sodium deoxyribonucleinate; 1 ml of solution contains 15 mg of the active substance. The solution also contains additional components, which include sodium chloride and water for injection.

Solution for injection Derinat is contained in glass bottles of 2 and 5 ml. They are placed in special blister trays of 5 and 10 bottles. The cardboard pack contains one blister tray and instructions for use of the drug.

pharmachologic effect

The active ingredient of the drug Derinat has a number of pharmacological effects by stimulating and optimizing the functioning of the cellular and humoral components of the immune system. These effects include:

  • Stimulation of the functional activity of the main cells of the immune system - T-helpers, B-lymphocytes, macrophages.
  • Improving the energy metabolism of all cells of the body, increasing the intensity of DNA and RNA synthesis processes.
  • Binding free radicals (fragments of organic molecules that have one unpaired electron) in the body and improving their elimination, which reduces their destructive effect on cell membranes.
  • Improving the drainage and detoxification function of the lymphatic system - increasing the removal of harmful products and foreign agents from tissues in the area of the inflammatory process.
  • Stimulating the processes of regeneration of damaged cells, in particular during the formation of stomach or duodenal ulcers.

Due to these pharmacological effects, the solution for parenteral administration Derinat stimulates a nonspecific protective reaction of the body during infectious processes caused by bacterial, viral or fungal infection. The drug also improves the nutrition of heart cells during coronary artery disease. The active ingredient of the drug stimulates hematopoiesis - the process of formation and maturation of blood cells in the red bone marrow.

After intramuscular injection of Derinat solution, sodium deoxyribonucleinate accumulates in the blood quite quickly to a therapeutic concentration (usually within minutes). The active substance distributes well throughout the body with a predominant accumulation in lymphoid tissue and red bone marrow. It penetrates through the placental barrier into the body of the developing fetus during pregnancy and through the blood-brain barrier into the structures of the central nervous system of an adult. The active substance of the drug is metabolized in the cells of the human body. Decomposition products (metabolites) are excreted from the body in the urine by the kidneys, and in small quantities with feces. The half-life (the period of time during which 50% of the entire dose of the administered drug is eliminated from the body) is about 72 hours.

Indications for use

The use of a solution for parenteral administration of Derinat is indicated for a number of pathological conditions of the body, which are accompanied by a violation of the functional activity of the immune system or other systems. These include:

  • Acute respiratory viral infections (ARVI), which have a severe or complicated course (the addition of a bacterial infection leading to bronchitis or pneumonia).
  • As part of complex therapy for chronic inflammatory diseases of various localizations that do not respond to standard therapy.
  • Chronic obstructive pulmonary pathology, which is accompanied by periodic bacterial complications.
  • Complex treatment of infectious diseases caused by various bacteria, viruses or fungi.
  • Urogenital infectious pathology, which is caused by a specific infection (chlamydia, ureaplasma, mycoplasma, genital herpes virus or human papilloma).
  • To accelerate regenerative processes (tissue restoration) after major surgical interventions, burns or injuries.
  • Treatment of peptic ulcers, erosions of the duodenum or stomach.
  • Inflammatory processes of the female reproductive system (endometritis, chronic inflammation of the uterine appendages).
  • Benign prostatic hyperplasia (prostate adenoma) or prostatitis (inflammation) in men.
  • Trophic ulcers are a long-term non-healing violation of the integrity of the skin and subcutaneous tissue in the form of erosion, which is the result of impaired tissue nutrition.
  • Infectious and inflammatory complications, the development of which is caused by long-term use of cytostatic drugs (in particular stomatitis - chronic infectious inflammation of the oral mucosa).
  • Pathology of peripheral arteries with impaired blood flow in them (obliterating endarteritis or atherosclerosis of the vessels of the lower extremities).
  • Complex therapy of coronary heart disease.
  • Treatment of radiation injury or radiation sickness.
  • Pulmonary tuberculosis.
  • Pathology of the blood system and red bone marrow, accompanied by disruption of the process of hematopoiesis.
  • Secondary immunodeficiency (insufficient functional activity of the immune system) of any origin.

Derinat injection solution is also used in the complex treatment of cancer patients who have been receiving radiation treatment or chemotherapy for a long time.

Contraindications for use

Administration of Derinat solution is contraindicated in the presence of individual intolerance, hypersensitivity to the active substance or auxiliary components of the drug.

Directions for use and doses

Solution for injection Derinat is administered intramuscularly. The recommended dose of the drug for a single administration is on average 75 mg (5 ml), the administration interval is an hour. The dosage and regimen of the drug may differ depending on the pathological process:

  • Acute infectious pathology - on average 3-5 injections with an interval of 24 hours (once a day).
  • Chronic infectious pathology - 5 injections with an interval of 24 hours (once a day), then another 5 injections with an interval of 72 hours (once every 3 days).
  • Gynecological pathology – 10 injections at hourly intervals.
  • Inflammatory or infectious pathology of the urogenital tract in men - 10 injections at an hourly interval.
  • Peptic ulcer of the duodenum or stomach - 10 injections with an interval of 48 hours (once every 2 days).
  • Oncological pathology (benign or malignant neoplasms) - depending on the severity and severity of the process, the solution is administered 3 to 5 times with an hourly interval.
  • Pulmonary tuberculosis - injections at hourly intervals.

For children, the drug administration regimen is the same. For children under 2 years of age, a single dose is 7.5 mg (0.5 ml); for children aged 2 to 10 years, the dose is calculated based on 0.5 ml per 1 year of the child’s life. For children over 10 years of age, the single dose of the drug is the same as for adults; the maximum course of treatment for them is 5 injections of the drug.

Reviews

We have selected some people's reviews about the drug Derinat:

  1. Anna . No matter how much I use Derinat for a runny nose, for some reason it doesn’t help me. Doctors praise it, there are a lot of positive reviews about it, but for me it is completely useless. I took several courses of treatment for colds, but did not feel any effect. It is possible that my body simply does not perceive its healing properties. A placebo of sorts. For colds, I prefer IRS-19; it is a similar analogue of Derinat, as the pharmacy explained to me.
  2. Lola. This is the third autumn before the onset of cold weather that I have been buying Derinat for my children and taking preventive measures to strengthen their immunity and protect them from viruses so that they don’t catch anything. The drug is good, there is nothing complicated to use, just put some drops in your nose in the morning, afternoon and evening. Well, follow the hygiene rules! But for two autumns my children have not been sick and attend school and kindergarten normally. For me it’s better to foresee everything in advance, prepare and then be calm.
  3. Svetlana . After the flu, I was given Derinat injections to prevent and strengthen the immune system. The runny nose went away quickly, my throat didn’t hurt, I felt better. The injections themselves are unpleasant, but I won’t argue that they are effective. The price is affordable, buying at the pharmacy is not a problem. Since the composition is natural, there is no need to select an analogue - everything works.
  4. Galina. These drops are good because they can be used not only for treatment, but also for the prevention of ARVI. To do this, they are dripped according to the scheme of 2 drops 3 times a day for one to two weeks. But if you have already felt the symptoms of a cold, then you need to instill 2-3 drops every 2 hours on the first day you get sick. And then two or three drops 3 times a day. Available in pharmacies without a prescription, they are suitable even for infants. A very good product to support your immune system.

Analogs

Pharmacies offer a number of Derinat analogues that have similar pharmaceutical effects: Deoxynate, Sodium Deoxyribonucleate, Panagen. These analogues also have pronounced regenerative properties, helping the body cope with bacterial, viral or fungal agents.

Before using analogues, consult your doctor.

Storage conditions and shelf life

Store in a dry place, protected from light, out of reach of children at a temperature of 4-20 °C.

Shelf life – 5 years.

The contents of an opened bottle of solution for topical and external use should be used within 14 days.

Be careful when using the drug in children. I am an adult, the ENT doctor first prescribed inhalations with Derinat, then a throat spray. More for preventive purposes (the throat often hurts). I did inhalations for 3 days, now I have been using the spray for the 4th day. The result is terrible! Sore throat, cough, sore throat. Before using it, my throat just hurt, but now everything is swollen. Bad drug! Definitely. Do not use in children.

I can’t agree, Natasha, because I’ve also been using derinat for a long time and, as a rule, with (+) results. The spray perfectly helped relieve pain and redness, although, in my opinion, if the process is very advanced (not at the beginning), then the spray alone will not help, you also need to add an antibiotic. And if only for the viral process, then you can use one. Perhaps you have an individual intolerance to the drug?

Derinat drops - method of use, instructions

General information about the drug

The active ingredient of this product is the sodium salt of deoxyribonucleic acid. It is DNA that is considered the carrier of the genetic code (or genetic information) of all cells of a living organism. The abbreviation for the name of the acid formed the basis for the name of the drug. The history of the creation of Derinat began in the sixties of the twentieth century. At that time, large-scale research work was carried out at the Institute of Biophysics of the USSR Ministry of Health - the radioprotective properties of the sodium salt of deoxyribonucleic acid were studied. At that time, various nuclear programs were rapidly developing in our country, which resulted in the appearance of patients exposed to radioactive rays. DNA was then obtained from marine animals. In the mid-eighties, our research scientist Yu. P. Weinberg created a special installation that made it possible to extract a valuable native substance from sturgeon milt. In 1994, the drug Derinat began to be produced on an industrial scale. It was registered in 1996. Then it was recommended as a radioprotector, immunomodulator and stimulator of hematopoiesis (normalizing the number of different types of white cells in the blood).

  • 5 ml bottles for intramuscular administration (1.5% concentration solution);
  • dark glass bottles containing 10 (or 20) ml of solution used externally (concentration 0.25%);
  • drops (10 ml of solution in dropper bottles; the package may also contain a spray nozzle).
Most often, drops and a solution for topical application are used. The international name of the active substance is sodium deoxyribonucleate, the auxiliary components are water for injection and sodium chloride. The drops are a transparent, colorless liquid that should not contain any inclusions.

Action of Derinat

First of all, the medicine affects two types of immunity:

  • cellular (activation of macrophage production);
  • humoral (it is characterized by the synthesis of antibodies in response to the arrival of foreign antigens in the body.

How does Derinat work?

  • Activates B-lymphocytes (these are white blood cells endowed with the ability to turn into cells that produce antibodies).
  • Activates T-helpers (helpers of the immune system).
  • Normalizes the number of phagocytes that “devour” pathogenic foreign agents, as well as the number of platelets, important participants

Pharmacotherapeutic group

Pharmacological properties

Derinat helps increase the body's resistance to infection, takes part in the regulation of hematopoiesis, stimulates the drainage and detoxification function of the lymphatic system, and also reduces the sensitivity of cells to the effects of chemotherapy drugs and radiation therapy.

The active component of the drug is quickly distributed in the body, has a high affinity for the organs of the hematopoietic system and is easily integrated into cellular structures.

When administered daily, Derinat accumulates in the bone marrow, lymph nodes, thymus and spleen, as well as other tissues of the body. The peak concentration of sodium deoxyribonucleate in the bone marrow is observed 5 hours after intramuscular injection of the solution, in the brain - after 30 minutes. Derinat easily penetrates the blood-brain barrier.

Derinat is excreted mainly by the kidneys, some part is excreted by the intestines.

Indications for use Derinat

Derinat injection solution is also prescribed for hematopoietic disorders, trophic ulcers and long-healing wounds, burn disease, rheumatoid arthritis, purulent-septic complications and odontogenic sepsis, as well as for patients with stomatitis, myelodepression and resistance to cytostatic agents in patients who receive cytostatic or radiation therapy.

Contraindications

Prescribing the parenteral form of the drug Derinat to women during pregnancy and lactation is not recommended.

The drug Derinat does not affect the reaction rate and can be used to treat patients whose work requires increased concentration.

Caution should be exercised when prescribing the drug to patients with diabetes.

Interactions with drugs

Derinat injection solution potentiates the effect of antitumor anthracycline antibiotics and cytostatic drugs.

Sodium deoxyribonucleate increases the effectiveness of basic drugs intended for the treatment of duodenal and gastric ulcers.

With the combined use of the drug Derinat and drugs used for surgical sepsis, patients experience activation of the body's defenses, a decrease in the level of intoxication, an improvement in the function of the hematopoietic system, as well as an improvement in the function of organs that regulate detoxification processes.

The drug should not be used topically at the same time as hydrogen peroxide.

Directions for use and dosage Derinat

To prevent acute respiratory infections, 2 drops of Derinat should be instilled into the nasal passages 2-4 times a day. The duration of prophylactic use of Derinat solution is 1-2 weeks.

For acute respiratory diseases, 2-3 drops of Derinat should be instilled into the nasal passages at intervals of 1-1.5 hours. Starting from the second day of therapy, they switch to instilling 2-3 drops of Derinat into the nasal passages 3-4 times a day. The duration of treatment is 2 weeks.

For inflammatory diseases of the nasal mucosa and paranasal sinuses, 3-5 drops of Derinat should be instilled into the nasal passages 3-6 times a day. In addition, instead of instilling the solution, you can moisten cotton swabs and insert them into the nasal passages 3-6 times a day. The duration of treatment is 1 month.

For various obliterating diseases (in particular the lower extremities), 1-2 drops of Derinat are instilled into the nasal passages up to six times a day. The duration of treatment ranges from 1 to 6 months.

For gangrene, trophic ulcers, burns and long-healing wounds, Derinat is used as applications. To do this, 2 layers of gauze are moistened with the solution and applied to the affected area of the skin three times a day. To achieve the maximum therapeutic effect, the use of applications is supplemented by instillation of Derinat solution into the nasal passages (1-2 drops into the nasal passages up to six times a day). Duration of treatment is 1-3 months.

For diseases of the rectum, Derinat is used in the form of microenemas (15-40 ml of solution). The duration of treatment is 4-10 days.

In ophthalmic practice, Derinat is used in the form of eye drops, instilling 1-2 drops into the conjunctival sac twice or thrice a day. The duration of treatment is from 2 to 6 weeks.

For adults, as a rule, the drug Derinat is prescribed in a single dose of 5 ml.

Adults with coronary heart disease, gynecological and andrological diseases are prescribed a single dose every hour. The course dose is 10 injections.

Adults with cancer are prescribed a single dose every hour. The course dose ranges from 3 to 10 injections.

Adults with pulmonary tuberculosis are prescribed a single dose every hour. The course dose is injection.

Adults with chronic inflammatory processes are prescribed a single dose every 24 hours, after 5 days they switch to a single dose every 72 hours. The total course dose is 10 injections.

A single dose for children under 2 years of age is 0.5 ml of Derinat solution.

A single dose for children aged 2 to 10 years is 0.5 ml of Derinat solution for each year of life.

A single dose for children over 10 years of age is 5 ml of Derinat solution.

Side effects

In addition, some patients experienced an increase in body temperature of up to 38 degrees Celsius within 1.5-3 hours after injection of sodium deoxyribonucleate. The development of hyperthermia does not require discontinuation of the drug; to normalize body temperature, symptomatic drugs are prescribed (non-steroidal analgesics-antipyretics, diphenhydramine).

Hypoglycemia was observed predominantly in patients with diabetes mellitus when using the drug Derinat.

The composition of 1 ml of solution for local and external use includes:

  • Active substance: sodium deoxyribonucleate – 2.5 mg;
  • Auxiliary components: sodium chloride, water for injection.

Pharmacological properties

Pharmacodynamics

Derinat activates the processes of humoral and cellular immunity. The immunomodulatory effect is achieved through stimulation of B lymphocytes and activation of T helper cells. The drug activates the body's nonspecific resistance, optimizes inflammatory reactions, as well as the immune response to viral, fungal and bacterial antigens. Helps stimulate regenerative and reparative processes. Increases the body's resistance to infections, regulates hematopoiesis (ensures normalization of the number of lymphocytes, leukocytes, granulocytes, platelets, phagocytes).

Due to its pronounced lymphotropic properties, taking Derinat stimulates the drainage and detoxification functions of the lymphatic system. The drug significantly reduces the sensitivity of cells to the effects of radiation therapy and chemotherapy drugs. Does not have embryotoxic, teratogenic or carcinogenic effects.

Pharmacokinetics

It is quickly absorbed and distributed in tissues and organs along the endolymphatic transport route. It has a high affinity for the organs of the hematopoietic system, is integrated into cellular structures, due to which it actively participates in cellular metabolism. In the phase of intensive entry into the blood, in parallel with the processes of metabolism and excretion, the drug is redistributed between the blood plasma and its formed elements. After a single injection, in all pharmacokinetic curves of changes in the concentration of sodium deoxyribonucleate in the studied tissues and organs, rapid phases of increasing and decreasing concentrations are observed in the time interval from 5 to 24 hours. When administered intramuscularly, the half-life is 72.3 hours.

It is quickly distributed in the body, and during the daily course of treatment accumulates in tissues and organs (mainly in the lymph nodes, bone marrow, thymus, spleen). To a lesser extent, the drug accumulates in the brain, liver, stomach, large and small intestines. The time to reach maximum concentration in the bone marrow is 5 hours, and in the brain – 30 minutes. Penetrates the blood-brain barrier.

Metabolized in the body. It is excreted biexponentially in the form of metabolites in the urine and, to a lesser extent, in feces.
