At the same time, the resistance of bacteria to the action of the body’s protective immune mechanisms decreases. The combination of these mechanisms leads to a decrease in the activity of inflammatory processes and the elimination of pathogenic agents from the body. This effect in pharmacology is called bacteriostatic.
The unique features of macrolides are their long-term presence in the body in therapeutic concentrations.
Many studies have shown that they have the ability to accumulate in body tissues (especially in the respiratory epithelium at the source of inflammation). Also, antibiotic molecules pass into the cells of the immune system, with which they are transported to the site of the pathological process in the body.
After taking the drug, the concentration of midecamycin in tissues may immediately exceed the amount of antibacterial agent in the blood. Moreover, the effect of the drug lasts 72 hours after the time of its administration, which makes it possible to reduce the duration of the course of treatment.

After administration, it is quickly and completely absorbed from the gastrointestinal tract. The maximum concentration of the drug in the serum is after 1–2 hours. At this level, the concentration remains for up to 6 hours. Eating slightly reduces the concentration of the drug in the blood, so it is recommended to take it before meals. Children weighing less than 30 kg - 20–40 mg/kg, dividing the daily dose into 3 times. The daily dose for severe infections can be 50 mg/kg, also divided by 3 times.

This form of release is used in childhood. Depending on the child’s body weight, the dosage is calculated and taken 2 times a day:
- from 0 to 5 kg -3.75 ml of suspension 2 r/s;
- from 5 to 10 kg -7.5 ml of suspension 2 r/s;
- from 10 to 15 kg - 10 ml of suspension 2 r/s;
- from 15 to 20 kg - 15 ml of suspension 2 r/s;

The duration of treatment is from 7 to 14 days, for the treatment of chlamydial infections - 14 days.
To prevent diphtheria, the drug is prescribed at a dose of 50 mg/kg per day, divided into 2 doses, for 7 days. A control bacteriological study after completion of therapy is recommended.
To prevent whooping cough, the drug is prescribed at a dose of 50 mg/kg per day for 7-14 days in the first 14 days from the moment of contact.
To prepare the suspension, add 100 ml of boiled or distilled water to the contents of the bottle and shake well. It is recommended to shake the prepared suspension before use.

It is effective against intracellular microorganisms: Mycoplasma spp., Chlamydia spp., Legionella spp., Ureaplasma urealyticum; gram-positive bacteria: Stretococcus spp., Staphylococcus spp., Corynebacterium diphtheriae, Listeria monocytogenes, Clostridium spp. and some gram-negative bacteria: Neisseria spp., Moraxella catarrhalis, Bordetella pertussis, Helicobacter spp., Campylobacter spp., Bacteroides spp.
After oral administration, the drug is quickly and fairly completely absorbed from the gastrointestinal tract (GIT). Maximum serum concentrations of midecamycin and midecamycin acetate are 0.5-2.5 mcg/L and 1.31-3.3 mcg/L, respectively, and are achieved 1-2 hours after oral administration.
High concentrations of midecamycin and midecamycin acetate are created in internal organs (especially in lung tissue, parotid and submandibular glands) and skin. The minimum inhibitory concentration (MIC) persists for 6 hours. The half-life is approximately 1 hour. Protein binding - 47% midecamycin and 3-29% metabolites.
The drug is metabolized in the liver to form 2 active metabolites with antimicrobial activity. It is excreted in the bile and to a lesser extent (about 5%) by the kidneys.
