Release form, composition and packaging

The solution for injection and inhalation is transparent, colorless, with a faint sulfuric odor; After opening the ampoule, a faint pink-violet tint may appear upon prolonged contact with air.

Table of contents:

Excipients: disodium edetate, sodium hydroxide, water for injection.

3 ml - dark glass ampoules (5) - plastic holders (1) - cardboard packs.

Clinical and pharmacological group

pharmachologic effect

Mucolytic drug. Liquefies mucus, increases its volume and facilitates its separation. The action of acetylcysteine is associated with the ability of its sulfhydryl groups to break intra- and intermolecular disulfide bonds of acidic mucopolysaccharides of sputum, which leads to depolarization of mucoproteins and a decrease in sputum viscosity. Remains active in the presence of purulent sputum.

Increases the secretion of less viscous sialomucins by goblet cells, reduces the adhesion of bacteria to the epithelial cells of the bronchial mucosa. Stimulates mucous cells of the bronchi, the secretion of which is lysed by fibrin. It has a similar effect on the secretions formed during inflammatory diseases of the ENT organs.

It has an antioxidant effect due to the presence of an SH group that can neutralize electrophilic oxidative toxins.

Acetylcysteine easily penetrates into the cell and is deacetylated to L-cysteine, from which intracellular glutathione is synthesized. Glutathione is a highly reactive tripeptide, a powerful antioxidant and cytoprotector that neutralizes endogenous and exogenous free radicals and toxins. Acetylcysteine prevents exhaustion and helps increase the synthesis of intracellular glutathione, which is involved in the redox processes of cells, promoting the detoxification of harmful substances. This explains the effect of acetylcysteine as an antidote for paracetamol poisoning.

Protects alpha1-antitrypsin (elastase inhibitor) from the inactivating effects of HOCl, an oxidizing agent produced by myeloperoxidase of active phagocytes. It also has an anti-inflammatory effect (by suppressing the formation of free radicals and reactive oxygen-containing substances responsible for the development of inflammation in the lung tissue).

Pharmacokinetics

When taken orally, it is well absorbed from the gastrointestinal tract. With intravenous administration of 600 mg of acetylcysteine, Cmax in plasma is 300 mmol/l.

Penetrates into the intercellular space, predominantly distributed in the liver, kidneys, lungs, and bronchial secretions. Penetrates through the placental barrier.

Vd at equilibrium is 0.34 l/kg.

Rapidly deacetylated to cysteine in the liver. In the plasma, a mobile equilibrium of free acetylcysteine and its metabolites (cysteine, cystine, diacetylcystine) is observed, free and bound to plasma proteins.

T1 /2 is 2 hours. It is excreted in the urine in the form of inactive metabolites (inorganic sulfates, diacetylcysteine), a small part is excreted unchanged through the intestines.

Indications for use of the drug

- diseases of the respiratory system, accompanied by impaired sputum discharge (including bronchitis, tracheitis, bronchiolitis, pneumonia, bronchiectasis, cystic fibrosis, lung abscess, pulmonary emphysema, laryngotracheitis, interstitial lung diseases, pulmonary atelectasis /due to blockage of the bronchi by mucus plug/) ;

- catarrhal and purulent otitis media, sinusitis, incl. sinusitis (to facilitate the passage of secretions);

— to remove viscous secretions from the respiratory tract in post-traumatic and postoperative conditions;

— preparation for bronchoscopy, bronchography, aspiration drainage;

- for washing abscesses, nasal passages, maxillary sinuses, middle ear, treatment of fistulas, surgical field during operations on the nasal cavity and mastoid process.

Dosage regimen

For aerosol therapy, 3-9 ml of a 10% solution is sprayed in ultrasound devices, and 6 ml of a 10% solution is sprayed in devices with a distribution valve. Duration of inhalation min; multiplicity times/day When treating acute conditions, the average duration of therapy is days; for long-term therapy of chronic conditions, the course of treatment is up to 6 months.

Due to the high safety of the drug, the frequency of administration and the relative dose can be changed by the doctor within acceptable limits, depending on the patient’s condition and the therapeutic effect. There is no need to change the adult dose when using the drug to treat children. In the case of a strong secretolytic effect, the secretion is sucked out, and the frequency of inhalations and the daily dose are reduced.

To wash the bronchial tree during therapeutic bronchoscopy, use 1-2 ampoules or more per day, depending on clinical indications.

Instill into the external auditory canal and nasal passages (1.5-3 ml) for 1 procedure.

Administer intravenously (preferably drip or slow stream over 5 minutes) or intramuscularly. Adults - 300 mg (3 ml) 1-2 times a day; children from 6 to 14 years old - 150 mg (1.5 ml) 1-2 times a day. The daily dose for children under 6 years of age is 10 mg/kg body weight; for children under 1 year of age, intravenous administration of acetylcysteine is possible only for health reasons in a hospital setting. The duration of treatment should be determined based on changes in the patient's condition.

It should be taken into account that oral administration of the drug is preferable for children under 6 years of age.

High local and general tolerability of the drug allows for long courses of treatment.

For intravenous administration, the solution is further diluted with 0.9% sodium chloride solution or 5% dextrose solution in a 1:1 ratio. The duration of therapy is determined individually (no more than 10 days).

In patients over 65 years of age, the minimum effective dose is used.

Side effect

With aerosol therapy: reflex cough, respiratory tract irritation, rhinorrhea; rarely - stomatitis, bronchospasm.

With intramuscular injection: burning at the injection site, skin rash, urticaria; with long-term treatment - impaired liver and/or kidney function.

Contraindications to the use of the drug

- peptic ulcer of the stomach and duodenum in the acute phase;

- hypersensitivity to acetylcysteine.

with caution for peptic ulcers of the stomach and duodenum in remission, varicose veins of the esophagus, hemoptysis, pulmonary hemorrhage, bronchial asthma (with intravenous administration the risk of developing bronchospasm increases), diseases of the adrenal glands, liver and/or kidney failure, arterial hypertension , as well as children under 6 years of age.

For children under 1 year of age, intravenous administration of acetylcysteine is possible only for health reasons in a hospital setting.

Use of the drug during pregnancy and lactation

During pregnancy, the use of Fluimucil is possible only if the expected benefit to the mother outweighs the potential risk to the fetus.

If it is necessary to prescribe the drug during lactation, breastfeeding should be stopped.

Use for liver dysfunction

The drug should be prescribed with caution to patients with liver disease.

Use for renal impairment

The drug should be prescribed with caution to patients with kidney disease.

special instructions

For patients with bronchial asthma and obstructive bronchitis, acetylcysteine should be prescribed with caution under systematic monitoring of bronchial patency.

With a shallow intramuscular injection of the drug and in the presence of increased sensitivity, a slight and quickly passing burning sensation may appear, and therefore it is recommended to inject the drug deep into the muscle.

The Fluimucil ampoule is opened before use. An opened ampoule can be stored in the refrigerator for 24 hours, during which time the drug can only be used for inhalation.

Fluimucil solution should not come into contact with rubber and metal surfaces.

Overdose

Fluimucil ® when taken at a dose of 500 mg/kg/day does not cause signs and symptoms of overdose.

Drug interactions

The simultaneous use of Fluimucil with antitussives may increase sputum stagnation due to suppression of the cough reflex.

When used simultaneously with antibiotics, such as tetracyclines (excluding doxycycline), ampicillin, amphotericin B, they may interact with the thiol group of acetylcysteine, which leads to a decrease in the activity of both drugs, so the interval between taking acetylcysteine and antibiotics should be at least 2 hours.

The simultaneous use of Fluimucil and nitroglycerin can lead to an increase in the vasodilatory effect of the latter.

Acetylcysteine reduces the hepatotoxic effect of paracetamol.

Pharmaceutically incompatible with other drug solutions.

Conditions for dispensing from pharmacies

The drug is available with a prescription.

Storage conditions and periods

The drug should be stored out of the reach of children at a temperature of 15° to 25°C. Shelf life: 5 years.

    How to put Fluimucil drops into your nose?

    Everyone knows that a stuffy nose can reduce the quality of life and cause a number of undesirable consequences. In addition to the loss of smell, the patient may develop sinusitis, acute rhinitis, inflammation of the respiratory tract and other ailments that will affect the patient’s health. When fighting such diseases, comprehensive treatment is necessary.

    When treating a runny nose and cough, many patients use a variety of medications. But not many people know that there is an effective drug that reduces the symptoms of inflammation in a short time - Fluimucil.

    About the composition and structure of the medicine

    Fluimucil nasal spray is a well-known Swiss medicine for relieving symptoms of inflammation in the respiratory tract.

    The active component of the drug is acetylcysteine. This is a white or slightly yellow component, made in the form of a crystalline powder. Easily dissolves in water and purified alcohol.

    Acetylcysteine is used as an expectorant, mucolytic, and detoxifying substance.

    This effect of the drug is due to disulfide bonds, which dilute sputum and mucous secretions.

    The pharmacological effect occurs due to an increase in the volume of mucous secretions. Thus, stimulation of mucosal cells occurs, which produce secret fluid. Having a stimulating effect on these processes, there is an outflow of liquid secretions.

    In addition, the active substance of the spray allows for an anti-inflammatory effect. This action is possible due to the suppression of the formation or development of radicals, which are responsible for the process of inflammation in the body.

    When taken, acetylcysteine is rapidly absorbed, but patients should be warned about its low bioavailability. It does not exceed ten percent during initial use. The effect of the drug, which contains acetylcysteine, is achieved after three hours.

    "Flumicil" for sinusitis is active not only in the case of mild symptoms of inflammation, but also in case of purulent discharge.

    In addition, nasal drops with acetylcysteine prevent the penetration of bacteria into the mucous membrane and normalize the functioning of the epithelium, and also have a stimulating effect on synthesizing substances that break down fibrin threads.

    The active component of the drug has a similar effect in case of damage to the upper respiratory tract.

    Fluimucil can be used for a runny nose, as well as in case of the following ailments:

    • to stimulate the respiratory tract;
    • purulent formation in the middle part of the ears;
    • otitis media;
    • acute inflammation of tracheitis;
    • pneumonia;
    • acute bronchiolitis;
    • chronic inflammation of sinusitis;
    • inflammation of the maxillary region;
    • acute sinusitis;
    • laryngotracheitis;
    • bronchitis;
    • sinusitis;
    • bronchial asthma.

    Fluimucil can be used for various operations in the upper jaw or nasopharynx.

    For the listed ailments, the medicine can be used only after consultation with the attending physician, since the drug has certain contraindications.

    Contraindications

    After determining the nature of the disease, it is necessary to determine all the signs and symptoms.

    Based on them, as well as after a comprehensive study of inflammation, the doctor will determine contraindications.

    If any of the following symptoms are present, taking the medication is prohibited.

    • inflammation of stomach and duodenal ulcers;
    • hypersensitivity to the active component of the drug;
    • When feeding your baby with breast milk, you should refrain from using the drug.

    The drug should not be used to treat children under two years of age. In the case of using Fluimucil tablets, the period of prohibition on using the drug reaches adulthood.

    In case of severe inflammation and the need to take Fluimucil when treating children, the doctor may prescribe the use of a solution of granules.

    In case of stomach ulcers or asthma inflammation, treatment should be carried out under the constant supervision of a doctor. Use the drug for hypertension or bleeding in the lungs with extreme caution, since the medicine has certain side effects with a long course of treatment.

    Do not use the product in case of renal and liver failure, as well as varicose veins in the esophagus.

    Side effects

    Side effects when taking medication occur only in case of an incorrect diagnosis or an incorrectly chosen dosage.

    dIn this case, the patient suffers from nausea, vomiting, dizziness, a feeling of heaviness in the abdomen, intestinal upset, heartburn and diarrhea.

    Manifestations of various allergic reactions are possible. In this case, patients complain of the formation of rashes on the skin, the appearance of hives and severe itching.

    Often there is a narrowing of the bronchi caused by muscle contraction in response to prolonged use of the medication.

    Very rarely, patients complained of the presence of extraneous noise immediately after taking the drug, as well as the appearance of nosebleeds and sudden cardiovascular failure, with loss of consciousness and the threat of death.

    Interaction

    When consulting with your doctor, you should indicate all the medications you are currently taking.

    Keep in mind that the simultaneous use of several cough medications can cause stagnation of mucous secretions.

    Such a negative symptom causes not only a constant cough, but also nasal congestion, as well as other extremely unpleasant factors.

    Do not use Fluimucil simultaneously with tetracyclines. This use can lead to a decrease in the effect of antibiotics and the active component of the spray. in the case of complex treatment, a break between medications is required for two hours. Remember that an antibiotic is necessary for sinusitis, since acute inflammation can only be gotten rid of using this method.

    If you have been prescribed medications that contain nitroglycerin, keep in mind that the vasodilator effect may be enhanced.

    Separately, it should be noted that the active substance of the drug “Fluimucil” reduces the toxic effect of paracetamol on the liver.

    special instructions

    Only the attending physician can prescribe Fluimucil, despite the fact that the drug is available without a prescription.

    When taking medication for bronchial asthma, you must strictly follow all the doctor’s advice, since this disease cannot be quickly treated.

    Experts give the same advice when treating bronchitis.

    How to carry out treatment correctly

    The instructions for using Fluimucil nasal drops indicate that the drops should only be used topically. For more complex diseases, the doctor may prescribe the drug intramuscularly or in the form of inhalations.

    Treatment of sinusitis

    In case of a runny nose, the remedy should be used as follows.

    First, cleanse your nasal passages with solutions prescribed by your doctor.

    Remember that treatment for adults is different from treatment for children. For the treatment of adults, inhalations are necessary.

    1. To do this, purchase a ten percent solution in an amount of three to nine milliliters.
    2. Carry out the procedure itself for fifteen minutes several times a day, but no more than three times.
    3. In case of chronic inflammation of rhinitis, treatment is necessary up to four times a day.
    4. Immediately after inhalation, clear your nose of mucus.

    The course of treatment should not exceed ten days. Five days are usually enough to treat a runny nose, but in the case of chronic inflammation, longer treatments are necessary. The exact number of days must be determined by the doctor.

    How to get rid of mucous discharge and cough with Fluimucil

    Use a nasal spray to treat a runny nose. Before introducing drops into the nose, it is necessary to perform sinus hygiene.

    To do this, rinse your nose with a solution of furatsilin or other medicinal components.

    Next, you need to spray the product into each nostril twice.

    Therapy should be carried out twice a day for one week. If the disease is more complex, complex treatment based on antibacterial drugs is necessary. But only the attending physician can prescribe them, so changing the course of treatment on your own is unacceptable.

    In case of inflammation of chronic runny nose and cough, it is necessary to use the product three times a day for no more than twelve days. If you do not notice the result on the fifth day of treatment, the medication is ineffective. In this case, it is necessary to reconsider the course of treatment with the help of a doctor.

    Conclusion

    It is important to note that treatment of people of retirement age must be carried out under the supervision of several doctors. This is due to the side factors of Fluimucil. If blood pressure changes or other side symptoms develop, serious deterioration in health may occur.

    When treating older people, the course of treatment is significantly shorter, as is the number of applications.

    Tablets in a package of 10 and 20 pcs.

    Oral solution in a bottle of 100, 150 and 200 ml.

    pharmachologic effect

    Liquefies mucus. The presence of sulfhydryl groups in the structure of acetylcysteine promotes the rupture of disulfide bonds of acidic mucopolysaccharides of sputum, which leads to depolymerization of mucoproteins and a decrease in mucus viscosity. The drug remains active in the presence of purulent sputum.

    Acetylcysteine has an antioxidant effect due to the presence of a nucleophilic thiol SH group, which easily donates hydrogen, neutralizing oxidative radicals.

    With the prophylactic use of acetylcysteine, there is a decrease in the frequency and severity of exacerbations in patients with chronic bronchitis and cystic fibrosis. The protective mechanism of acetylcysteine is based on the ability of its reactive sulfhydryl groups to bind chemical radicals.

    Acetylcysteine easily penetrates into the cell and is deacetylated to L-cysteine, from which intracellular glutathione is synthesized. Glutathione is a highly reactive tripeptide, a powerful antioxidant, cytoprotector that traps endogenous and exogenous free radicals and toxins.

    Acetylcysteine prevents exhaustion and helps increase the synthesis of intracellular glutathione, which is involved in the redox processes of cells, i.e. promoting the detoxification of harmful substances. This explains the effect of acetylcysteine as an antidote for paracetamol poisoning.

    Fluimucil is well absorbed when taken orally. It is immediately deacetylated to cysteine in the liver. In the blood there is a dynamic balance of free acetylcysteine and its metabolites (cysteine, cystine, diacetylcystine) bound to plasma proteins. Due to the high “first pass” effect through the liver, the bioavailability of acetylcysteine is about 10%. Acetylcysteine penetrates into the intercellular space and is predominantly distributed in the liver, kidneys, lungs, and bronchial secretions.

    After oral administration of 600 mg of acetylcysteine to healthy volunteers, Cmax in plasma is reached after approximately 1 hour and is 15 mmol/l, with intravenous administration - 300 mmol/l. T1 /2 from plasma - 2 hours. Acetylcysteine and its metabolites are excreted mainly by the kidneys.

    Respiratory diseases accompanied by the formation of high-viscosity sputum (acute and chronic bronchitis, pneumonia, bronchiectasis, cystic fibrosis, bronchial asthma).

    Contraindications

    Hypersensitivity to acetylcysteine, peptic ulcer of the stomach and duodenum in the acute stage, age under 18 years (effervescent tablets).

    Directions for use and doses

Children of the first year of life are given the resulting solution to drink from a spoon or feeding bottle. Children from 1 year to 2 years - 100 mg 2 times a day, from 2 to 6 years - 200 mg 2 times a day or 100 mg 3 times a day.

    The duration of treatment should be assessed individually. For acute diseases, the duration of treatment is from 5 to 10 days and up to several months for the treatment of chronic diseases (as recommended by a doctor).

    1 tablet is dissolved in 1/3 glass of water and taken 1 time per day.

    The duration of treatment should be assessed individually. For acute diseases, the duration of treatment is from 5 to 10 days and up to several months for the treatment of chronic diseases (as recommended by a doctor).

    Use during pregnancy and breastfeeding

    Use during pregnancy and breastfeeding is only possible if the intended benefit to the mother outweighs the potential risk to the fetus or child.

    Side effects

    When taken orally, in rare cases, nausea, heartburn, vomiting, diarrhea, skin rash, itching, urticaria, nosebleeds, and tinnitus are possible. Cases of the development of bronchospasm, collapse, stomatitis, and decreased platelet aggregation have been described.

    With parenteral administration - a slight burning sensation at the injection site, skin rash, urticaria.

    With inhalation administration - reflex cough, local irritation of the respiratory tract, stomatitis, rhinitis, rarely - bronchospasm (bronchodilators are prescribed).

    special instructions

    The drug contains aspartame, so it is not recommended for use in patients with phenylketonuria (granules, effervescent tablets).

    Drug interactions

    The combined use of acetylcysteine with antitussives may increase sputum stagnation due to suppression of the cough reflex.

    When used simultaneously with antibiotics such as tetracyclines (excluding doxycycline), ampicillin, amphotericin B, they may interact with the thiol group of acetylcysteine, which leads to a decrease in the activity of both drugs. Therefore, the interval between taking these drugs should be at least 2 hours.

    The simultaneous use of acetylcysteine and nitroglycerin can lead to an increase in the vasodilating and disaggregating effects of the latter.

    Acetylcysteine eliminates the toxic effects of paracetamol.

    Latest reviews of Fluimucil

    The drug is very good. I recommend Fluimucil! And for the price it's good!

    After taking fluimucil, a severe headache begins, the temperature rises, muscles ache, breathing becomes difficult, fever, and sensitivity to light increases. Lasts for several hours, then goes away. Probably individual intolerance.

    An excellent mucolytic. I tried many different drugs, but this one turned out to be the most effective for me.

    Fluimucil

    The drug Fluimucil is designed to effectively and quickly liquefy sputum. The strong mucolytic effect makes the drug indispensable in the treatment of inflammation of the respiratory tract with the concomitant formation of secretions of increased viscosity.

    Fluimucil's work is aimed at softening sputum, changing its volume and facilitating removal from the respiratory system. As a result, the patient’s condition is significantly alleviated and further treatment is more effective. The main active ingredient is acetylcysteine. The substance breaks intermolecular bonds in the secretion, thereby reducing its viscosity. The drug is active in the presence of purulent sputum. During treatment, there is an increase in less viscous sputum and a decrease in the viability of bacteria. The product has a sufficient antioxidant effect, neutralizing toxins. The product restores the normal course of redox processes in cells. If you have indications, then you can buy Fluimucil in Moscow cheaply and at a low price through an online store. ElixirPharm offers favorable purchases and reasonable prices in Moscow pharmacies.

    The peculiarity of the medicine is its ability to provide some anti-inflammatory effect by reducing inflammation in tissues. The drug is prescribed for the treatment of respiratory organs associated with difficult sputum discharge, such as tracheitis, pneumonia, emphysema, asthma, and so on. They treat purulent otitis, sinusitis, and sinusitis.

    Solution for injection and inhalation

    How to use Fluimucil:

    • parenterally: route of administration – intramuscular and intravenous (preferably drip or slow stream over 5 minutes); dosage regimen – 1–2 times a day, 3 ml (adults), 1–2 times a day, 1.5 ml (children 6–14 years old) or 10 mg/kg per day (children under 6 years old; it is preferable to prescribe the drug in oral forms; intravenous administration to children under 1 year of age is possible only if there are vital indications in a hospital setting);

    • intratracheal (during therapeutic bronchoscopy for washing the bronchial tree): 1–2 ampoules or more per day (depending on clinical indications).

    The duration of the course is determined by the results of treatment. Fluimucil has high general and local tolerability, so its long-term use is possible.

    Before intravenous administration, the solution should be further diluted with 5% dextrose solution or 0.9% NaCl solution (ratio 1:1).

    For patients over 65 years of age, Fluimucil is usually prescribed in minimally effective doses.
