Thiamphenicol has a wide spectrum of action on bacteria that cause respiratory tract infections - both gram-positive (streptococci, clostridia, staphylococci) and gram-negative (neisseria, Haemophilus influenzae, salmonella, yersinia, E. coli).
Acetylcysteine actively dilutes sputum and purulent discharge, promotes sputum discharge, reducing viscosity.
Acetylcysteine promotes better penetration of thiamphenicol into the lung tissue and prevents the adhesion of microorganisms to the mucous membrane of the respiratory system.
Pharmacodynamics and pharmacokinetics
Thiamphenicol accumulates in the respiratory tract, and maximum concentration occurs within 1 hour after injection into the muscle. The half-life is 3 hours, excreted in the urine.
Acetylcysteine accumulates in the intercellular space, bronchial secretions, lung tissue, liver and kidneys. Half-life – 2 hours; excreted by the kidneys, and a small part through the intestines.
Indications for use
- damage to the ENT organs and upper respiratory tract (sinusitis, otitis, laryngotracheitis);
- diseases of the lower respiratory system (bronchitis, lung abscess, pneumonia, bronchiectasis, cystic fibrosis, emphysema, whooping cough, bronchiolitis);
- treatment and prevention of infectious and obstructive complications after tracheostomy, thoracic surgery (atelectasis, bronchopneumonia);
- preparation for aspiration of bronchial secretions and bronchoscopy;
- in order to improve drainage in case of concomitant pulmonary infections (including tuberculosis, the presence of cavernous lesions).
Contraindications
- diseases and changes in blood count (leukopenia, anemia, thrombocytopenia);
- individual intolerance to substances included in the drug.
Kidney function in children of the first 2 years of life has features associated with age, therefore, in them, as well as in patients with severe damage to the liver and kidney functions, the drug must be used with caution.

Allergic reactions.
When injected into a muscle, there may be a burning sensation at the injection site; rarely leukopenia, thrombocytopenia, anemia, neutropenia, reticulocytopenia.
Using the antibiotic Fluimucil for inhalation can cause irritation of the respiratory mucosa, reflex cough, nausea, and rhinitis. Sometimes bronchospasm occurs, and bronchodilators should be used.
Instructions for use Fluimucil - IT antibiotic (Method and dosage)
Use intramuscularly in the dose:
- for adults, it is administered daily in 3 doses;
- children under three years of age 125 mg twice daily;
- children from 3 to 7 years mg twice a day;
- for children: 750 mg per day, divided into 3 injections.
For newborns up to 2 weeks, as well as premature infants, use a dose of 25 mg/kg body weight per day.
How to dilute for inhalation?
For adults, 500 mg of dry substance is dissolved in 4 ml of water for injection and divided into 2 doses per day (2 ml each). Inhalations for children are prepared in the same amount of water for injection as for adults (4 ml), dissolving 250 mg of powder and also used twice a day.
Locally for washing the paranasal sinuses, incl. after surgical interventions, for endotracheal administration through a tracheostomy, bronchoscope or endotracheal tube, the powder is dissolved using water for injection in an amount of 4 ml, 500 mg for adults and 250 mg for children.
For otitis and diseases of the nasopharynx, 2-4 drops are instilled into the nasal passage or ear canal.
In case of appropriate indications, the dose of the drug can be doubled.
In newborns, premature and elderly patients, do not increase the dose.
The instructions for the antibiotic determine the course of treatment with the drug for no more than 10 days.
Overdose
Possible increased side effects of the drug, superinfection, disturbances of the bacterial flora. Symptomatic therapy is used as treatment.
Interaction
If you use cough suppressants, mucus stagnation may occur.
It is not recommended to mix the inhalation antibiotic Fluimucil with other substances in an aerosol.

Fluimucil®-antibiotic IT is a combination drug; it combines an antimicrobial agent and a mucolytic that dilutes sputum.
What I like most about it is that it can be used as an inhalation, so it becomes a topical antibiotic, so it doesn't have many side effects. The solution can also be used for injection.
We used it for a child (3.5 years old). My daughter had a terrible cough, wet and practically unproductive; 3-day inhalations with Lazolvan did not help. Moreover, the cough was very intrusive, the child was exhausted.
The doctor prescribed this drug to us. By the way, I hope you will not self-medicate; only a doctor should prescribe such medications.
There was no wheezing in the lungs, just heavy breathing. The diagnosis is acute pharyngitis. The temperature lasted 3 days and reached 38.5.
We took the first inhalation in the evening, and the next day the cough ceased to be obsessive, a significant relief.

In the respiratory tract, the drug is broken down into thiamphenicol (its own antibiotic) and acetylcysteine (mucolytic). Acetylcysteine improves the transport of the antibiotic to the lower respiratory tract and to microbes located in lumps of mucus inside the bronchi. Thus, the effect of the antibiotic is enhanced.
Antibiotic
Thiamphenicol is a broad-spectrum antibiotic.
Its predecessor is chloramphenicol (chloramphenicol). According to the spectrum of action, thiamphenicol is close to chloramphenicol, but lacks its side effects (hepatotoxicity, inhibition of hematopoiesis, etc.)
Thiamphenicol is effective against the main bacterial pathogens of respiratory diseases in children: pneumococcus, streptococcus, Haemophilus influenzae, some strains of Staphylococcus aureus, Legionella, chlamydia, mycoplasma.
Thiamphenicol with this method of administration has less effect than other antibiotics on the intestinal microflora and does not irritate the stomach and intestines at all.
Mucolytic
Acetylcysteine remarkably thins mucus and helps its removal, improves the penetration of the antibiotic into the mucus and into the small bronchi and alveoli. But mothers must remember that when using acetylcysteine, in order for the medicine to be effective, the child needs to drink plenty of fluids.

- If fluimucil antibiotic is used to treat rhinosinusitis, rhinitis, the particle size should be greater than 10 microns.

- Particles of 0.5-2 microns are suitable for the treatment of pneumonia.

If laryngitis, tracheitis, bronchitis, pneumonia are being treated, it is better to breathe through the mouthpiece with your mouth.
Fluimucil antibiotic is sold in pharmacies in the form of a dry powder in bottles containing 500 mg of the main active ingredient. Each bottle comes with a 4 ml ampoule of water for injection. Before inhalation, water for injection is added to the bottle with the drug using a syringe.
After dissolving the powder, a solution with a volume of approximately 5 ml is obtained. The prepared solution can be stored in the refrigerator in a closed sterile bottle for 24 hours.
The therapeutic dose of the drug for children from birth to 12 years is 125 mg per inhalation 1-2 times a day. (In children under 2 years of age, the medicine is used with caution).
To receive this dose of the drug, you need to draw 1.25 ml of the resulting fluimucil solution from the bottle with a syringe and add 1 ml of physiological solution to it.
